Began as agency receptionist in 2006. Promoted to Regional Coordinator position in 2011.
Regional Coordinator over designated supportive care homes for individuals with Intellectual Disabilities and other diagnoses. Supervision duties include, but are not limited to:
oAssurance of quality and satisfaction of care to served persons.
oAssessed care needs of individuals served to write personal plans-of-care effectively. Touted for ability to provide extremely personalized and detailed care plans for those served.
oSupervisor of direct-care providers and homes, ensuring accuracy, accuracy and timeliness of Medicaid, licensure, DBHDS, and agency-required documentation by each care provider. Duties are deadline driven.
oResponsible to set and maintain monthly, quarterly, and annual appointments/meetings for individuals served, and ensuring necessary persons attend (guardians, staff, case management, individual served).
oCompletion of notes, data and reports regarding each person served and meeting/contact held, ensuring pertinent information is documented and handled effectively and promptly.
oUpdate file information for each person served, annually and as pertinent changes occur.
oRemain on-call to provide support and direction for crisis and emergent care. Support of those supervised on proper handling of such situations.
oRegular scanning, e-filing and hard-filing of all data and documentation.
