PROFESSIONAL EXPERIENCE

I most recently was the Director of Nursing and Assistant Administrator for Beloit Senior Living. I was responsible for all facets of resident care. I developed individual care plans and monitored progress. Held family conferences. Active in ensuring regulatory compliance. Managed resident care. In-serviced staff. Evaluated all admissions for appropriate levels. I retired in June 2022.

Previously employed at Alden Debes of Rockford. I was the Restorative Nurse. I enjoyed the job. I believe it is vital to keep residents moving to prevent debilitation.

I also have been employed as a nursing director at Rockford Supportive Living facility in Rockford Il. I oversee admissions, weights, admission and quarterly assessments, staffing and all aspects of resident care. I am on call 24/7. I supervise our Dementia Unit and am responsible for all staff training. I assess residents for admissions. We have 105 residents.

Atrium Memory Care- worked here for 3 months as Director of Nursing. Resigned due to extreme amount of hours at work with no relief plan. 11/17 to 2/18. I did enjoy the job however.

I was a nursing manager responsible for Restorative Programs and Wound Care. I also was involved in care planning, MDS, staff supervision, physician assessments, weights and worked as a staff nurse when needed.

ROCK RIVER BEHAVIORAL FACILITY

I was employed as a Behavioral nurse at Rock River Academy in Rockford ILL. I was employed there 12/2010 to 4/30/2015 . This facility closed April 30th, 2015. Phone- 815-***-****. This was a residential behavioral facility for adolescent girls. There were 60 girls. Responsible for not only medical but psychiatric care of girls.

AMBERWOOD HEALTHCARE CENTER – Rockford, Illinois 2009 - 2010 Director of Alzheimer Unit

Oversight of staff, nursing, psychotropic and medication management. Provided exceptional teaching and patient care and recognized patient’s conditions essential for the successful completion of the Alzheimer Unit. I was responsible for all staff seminars, social service/activities and admission assessments. I participated in MDS completion. Triaged with both physicians and psychiatrist. Instrumental in providing family/community education, training and support groups.

VITAL WELLNESS HOME HEALTH – Naperville, Illinois 2006 - 2009 Home Healthcare Nurse

Supplied skilled nursing care in a home setting. Performed patient assessments and community referrals. Provided complex feedback and recommendations to the attending physicians.
