Advance Practice-
*Geriatric/Long Term Care- transitions of care from acute care into post acute care facilities. Readmission rate reduction for ACO patients. Primary care and acute care management of residents in the facility. Review of labs, radiology exam, routine day to day care and acute management of resident needs.
*Cardiology/Vascular- NP experience in a busy office practice with NEXGEN EMR system. Pre/post procedure visits, new patients and follow up patients. Management of labs, diagnostics, and medications in collaboration with cardiologist. Subspecialty with pulmonary hypertension patients.
*Sleep Medicine NP- worked collaboratively to open Sleep Clinic and gain accreditation. Sole NP in practice with collaborating pulmonologist 1-2 days per week. New and follow up patient mix with medication and PAP therapy management.
