RELEVANT DIRECT SERVICE EXPERIENCES

Please note that I am currently providing hours of service part-time as a Co-Occurring Therapist as a Social Work Licensed Candidate.

2007 to Present: Access Recovery, Helena, Montana. Private practice.

For the past 16 years I have operated a small private practice in the Addictions and am currently providing supervised Co-Occurring services toward licensure as a LCSW. I provide biopsychosocial assessments, determine level of care needed per ASAM. If at level 1 then I provide addiction counseling. With my SWLC I also provide mental health counseling while under supervision during my internship. I provide a thorough Chemical Dependency Assessment that includes the Diagnostic and Statistical Manual 5 (DSM5); the Substance Abuse Subtle Screening Inventory (SASSI) and the American Society of Addiction Manual (ASAM) Diagnostic Interview. Additional instruments as appropriate. I provide also reports to the courts for DUI offenders. My philosophy of treatment includes holding no judgment against individuals which gives them permission to share as much as they can so appropriate treatments, referrals, etc. can be made.

2017 to 2020: Helena Indian Alliance, Helena, Montana: Post Graduate MSW Intern in Clinical Therapy in Social Work under Supervision by an LCPC for 1 Year, then by an LCSW for the 2nd year. Licensed Addiction Counselor. Providing Clients with Co-Occurring Disorders.

Provide biopsychosocial assessments, including Chemical Dependency Evaluations and follow up individual counseling that addresses and supports freedom from drugs or alcohol including lifestyle changes. Provide Anger Management Groups on Monday and Wednesday evenings. Started negotiations with the administration to start a men’s group for those who have graduated from WATCH and who remain sober. WATCH is a locked down facility for those who have committed their 4th DUI, which is a Felony in Montana. It treats criminal thinking based on Inside the Criminal Mind by Samenow, and The Criminal Mind in the Addictions also by Samenow. While the basis of treatment can seem dark, it is actually geared more to thinking errors that can lead to criminal thinking and behaviors. Additionally, it helps individuals develop new core beliefs that provide a design for living.

2006 to 2010: Licensed Addiction Counselor, Montana Chemical Dependency Center, Butte, MT, Level III.5-7 Intensive Inpatient for persons with co-occurring disorders.

Provided biopsychosocial intake assessments, individual and group counseling.

Conducted lectures in thinking errors and developing internal controls.

Wrote and documented continuing stay reviews and discharge plans.

Facilitated referrals to lesser level of care.

1995 to 2004: Administrative and Other Social Work Experience

Paul Wagler McCormick, Cont’d

1987 to 1995: Director, Division of Children’s Services, Child Care Administrator, Glenkirk, Northbrook, Il.

Administered 80 bed child care institution for children with developmental disabilities; private school; foster care; respite care; and adoption programs.

Developed curriculum based on Piaget’s developmental stages providing staff insight into the children under their care

Awarded highest level of accreditation by CARF.

1984-1987: Manager of Developmental Disabilities, a unit of the Division of Rehabilitation at Association House of Chicago.

Administered adult and adolescent therapeutic milieu care in residential homes, respite care, developmental training, case management and family support units.

1980-1984: Supervisor, Supported Living Arrangement, Association House of Chicago

Developed one of the first such residential programs in the State of Illinois, without a blueprint for program development. This was clinical and administrative. I had the opportunity to supervise staff in one of the first living arrangements of its kind in Chicago, Illinois. This was a result of President Ronald Reagan’s closing of all State operated institutions housing people with developmental and psychiatric disorders. We had 9 adults moving into a 3 flat apartment building in Humboldt Park, a neighborhood in Chicago’s lower north-west side. It was a port of entry for immigrants for over 100 years. A Settlement House, Association House of Chicago had its corner stone laid by Jane Addams herself in 1904. It served majority Puerto Rican population. The SLA that I supervised was a teachable environment for me. These awesome adults had never had their own bedroom, or access to one bathroom, a kitchen, living room, etc. My three staff and I wrote treatment plans in cooking and cleaning and working in the community, doing laundry in the community, going out to awesome events in Chicago, etc. They taught me more than I could have possibly taught them.

1978-1980: Case Manager for the Developmental Disabilities Unit of Association House of Chicago. This was old fashioned Social Work. All services provided in the community either at home or in schools and workshops. Advocating for social services and entitlements for families of children and adult children who had developmental disabilities. I am bilingual Spanish/English and none of my clients spoke English. I developed treatment plans for behavioral health of the client and family. It was during this tenure of my social service experience that I became fully bilingual in Spanish and English.
