I travel throught Pennsylvania providing dialysis treatments to patients in the hospital

setting. These patients are critically ill as most of the patients I dialyze are in the ICU.

In my current position I am responsible for helping to improve our IQI scores. When I began this position our IQI score was 3%. It is currently 55%. Some of the issues are not easily remedied, such as hospitalizations, but I have worked hard to obtain the monthly KT/V labs and increased the compliance from 60% to 99%. I have also assisted with educating the team regarding infection control, documentation, and educating the patients about fluid intake when they notice that a patient has gained large amounts of fluid. I do audits of the logs as well as the teammates. I audit patient charts and educate the team when I notice issues with documentation. I work closely with the Nephrologist and the CRNP regarding pt. issues including anemia, potassiums, pt. medications and target weights. I am also responsible for the monthly POC's and sending out the invitations to the team. I print the POC's and record them when they are completed. I am the anemia mananger for 160 patients. I review their labs, including Iron Sats and Ferritin. I assist on the clinic floor when needed and am available to assist the teammates in any capacity. I stress team work since I truly believe it is the only way to work to accomplish what is best for the patients.

I was responsible for Cat Audits in 31 hospitals throughout the west coast of Florida. I also taught the basic class for new PCT's and RN's each month. It was a week long class. I really enjoyed the education aspect. I wish I would have had that opportunity when I started in Dialysis. I precepted new teammates in the hospital working closely with them for 8 weeks. It was time consuming but rewarding. The audits were yearly and I was responsible for the Plan of Correction until the last few months when the FA's took that portion over. It was a very rewarding position but I relocated back to Pa. and the CNM position was offered to me.

10/2013 to 09/2016 Charge RN, PD RN and Acute dialysis RN Davita Dialysis Daytona Beach, Florida

As a PD RN I educated patients about PD including training, medication management, performed monthly labs and met with the patient and the Nephrologist on a monthly basis. I was responsible for 25 patients. As the Charge RN, I was responsible for patient assessments, medication administration, educating the patients about dialysis, diet, weight gains, etc., rounding with the MD, and treating any patients issues that arose. I was responsible for 24 patients in a clinic that has 3 turnovers a day. In the Acute care setting I was responsible for performing HD on patients in the hospital setting both stable and unstable.

I was a travel RN throughout the country in ER's and Dialysis both acutes and chronics. I had to be able to adapt to new environments quickly as most of the contracts were 13 weeks. I enjoyed it but decided to settle with my husband in Florida when he retired. I was also a Flight RN for a year on the Navajo Reservation. Without a doubt the best part of my nursing career.

Worked with patients throughout clinics in Philadelphia that were with Blue Cross and Blue Shield for their insurance. I resided in Harrisburg at the time and commuted 3-4 days a week. I do not recall the exact number of patients I was responsible for but it was between 90-100 in 15 -20 different clinics. My role was to meet with the patient and family and educate them about dialysis, dietary restrictions, lab results and there meaning, medication review and education, ways to monitor their weight between treatments. I was required to do home visits in the beginning and then yearly. The goal of the program was to keep the patient out of the hospital. I was also responsible for the computer documentation. I relocated and gave up the position.

02/2000 to 02/2004 Acute Dialysis Coordinator Davita Dialysis Harrisburg, PA Responsible for the daily operations of the acute care program at 2 hospitals in

the area. Was responsible for scheduling staff, updating nephrologists about their patients, changing assignments as needed according to the days activities, performing HD treatments, PD and CVVHD, on call 24/7.

Responsible for making the daily assignments in a 16 bed ICU/CCU. Also was a staff RN and recovered Open Heart patients as well as MI's and post-stent placements. Titrated medications, assisted with procedures such as chest tube placement, pacemaker placement and central line placements. The facility was purchased by another hospital and the services were transferred there.

Charge and staff RN for a 16 bed Cardiac Care Unit. Large teaching hospital that performed cutting edge procedures as well as experimental medication trials. Relocated and resigned.

02/1982 to 12/1984 Staff RN in a Medical Surgical ICU Frederick Memorial Hospital Frederick, MD

This was my introduction into the ICU arena. As the staff RN I was responsible for the care of immediate post-op ICU patients as well as patients with medical issues such as Heart Attacks, Strokes, severe pneumonia and sepsis. Relocated and resigned.

Responsible for the care of antepartum as well as active labor patients in a teaching hospital. Required fetal and maternal monitoring, assisting with deliveries as well as cesarean sections. Also responsible for recovering the patient after delivery. Relocated and changed my focus to ICU
