CLINICAL EXPERIENCE:

HOUSEJOB:

I completed one year of my house job in Liaquat National Hospital & Medical College (LNH&MC) Respectively. This is one of the largest private tertiary care institute with variety of patient in in-patient and out-patient departments. During the First six months I worked in Surgery and allied where I assisted routine ward rounds, Medical Emergencies, Neonatal surgical nursery and attended outpatient clinics. For the next six months, I rotated in Medicine and allied where I assisted in routine rounds, deal with emergency cases, attended outpatient Clinics of different sub-specialty in Pediatrics.

Pediatric Residency:

Experience as Junior Resident:

I have completed 4.5 year of residency in Liaquat national Hospital & Medical College (LNH&MC). During my junior years I got experience in general Pediatrics as well as different Pediatric Subspecialties. I got the opportunity to work in disciplines including neonatology where I performed multiple procedures including exchange transfusions, endotracheal intubations, chest tube and central lines insertions. I also took care of premature and extended premature neonates. I rotated in emergency medicine where I dealt with emergency procedures, resuscitation, acute critical care and Trauma patients.

I presented various clinical cases in weekly Case conferences, Patient care meetings, journal club and monthly morbidity and mortality meetings. Experience as Senior Resident:

During my third year of pediatric residency, I got an opportunity to do independent floater calls and lead all general and subspecialty teams on floor. I rotated in pediatric cardiology where I dealt with patients with congenital and acquired heart diseases. I also assisted cardiac catheterization procedures and spent significant amount of time in ECHO and outpatient clinics. I also worked in pediatric surgery where I attended patients in surgical wards, emergency room, Operation Theater and clinics. I presented a grand Round on Device Associated Healthcare Associated infection in Pediatric ICU.

In my fourth year, I worked in Pediatric Intensive Care Unit and Cardiothoracic Intensive Care. I was able to manage independently all acute and chronic critical care illnesses including Post-Operative congenital heart disease patients and I also assisted in critical care procedures including central lines insertion, intubations and Chest tube insertions. During my Fourth year, I also rotated in Pediatric emergency as Senior Resident. I had the chance to witness and manage wide variety of pediatric emergencies. I was part of the team performing various Pediatric Resuscitation during rotation.

Intermediate Module Exam (IMM):

I passed my Pediatric Fellowship Exam in 2018 from College of Physician and Surgeons of Pakistan (CPSP).

Experience of Pediatric ICU: As a Senior Resident

In Liaquat National Hospital being a Senior trainee, I have procured the following skills:

PICU: During Last Year of my Residency I have been trained to manage every kind of patients including patient with respiratory failure, status asthmatics, status epileptics, Guillain-Barre Syndrome, traumatic brain injury, post-operative brain tumors, myocarditis and cardiac failure, acute renal failure, Hemolytic Uremic Syndrome, complicated cases like Diabetic Ketoacidosis, thrombotic thrombocytopenic Purpura and patient with Poisoning (Organophosphorus, salt poisoning, carbamazepine poisoning ), . Besides being trained for Intubations, central lines (A-line, CVP) placement, Pigtail tail insertion for peritoneal and pleural fluids, I have also been trained for High frequency oscillatory ventilator (HFOV).

CICU: comprehensive CICU Team care is provided i.e. comprising of cardiac surgeon, Intensivist, cardiologist, anesthesia and trained pediatric cardiac nurses. During this training, I learned about pre and post-operative management issues like cardiovascular monitoring and pharmacology, ventilation, nutrition and acid base management, post-operative rhythm management i.e. atrial wire tracing, cardiac pacing, antiarrhythmic use etc. These patients have included neonates, infants, children and adults with congenital heart disease. I have managed preoperative and postoperative patients after common congenital heart diseases like Tetralogy of Fallot, Total anomalous pulmonary venous return defect, Ventricular Septal defect, Atrial septal defect, Patent ductus arteriosus, Coarctation, Palliative procedure like Blalock-Taussig shunt and Glenn shunt. I have done training under supervision of cardiac Intensivist, Cardiologist and pediatric cardiac surgeon

Experience in Pediatric Emergency:

During my Residency at Liaquat National I have been exposed to pediatric Emergency as Junior resident and as senior Resident as well. During my rotation, I have seen variety of Neonatal and pediatric emergencies including trauma, out of hospital cardiac arrest, various types of shock, dehydration, abdominal emergencies, neurologic emergencies, respiratory emergencies, metabolic, cardiac, oncological, hematological and endocrine emergencies. I was part of the team in resuscitating many pediatric cardiac arrests. I have done procedure including IV cannulations, endotracheal intubation, chest tube insertions, interosseous line insertion, etc. After completion of my post-graduate training, I have joined Child Life Foundation as Pediatric Emergency Physician and till date I am working here. Since Joining this organization, I am working in pediatric emergencies of Public Sector Teaching Hospital in Sindh, & Punjab Pakistan. There is a huge flow of patients in these pediatric emergencies, more than 1000,000 patient per month in 12 Pediatric Emergencies, with variety of illnesses. I have also completed the only Diploma Course Available in pediatric Emergency in Pakistan approved by Dow University of Health Sciences. In Child Life Foundation I got the chance to, not only, serve as pediatric emergency physician, but also, as a leader as Senior Clinical Instructor in Pediatric Emergency. I have been involved in Orientation, Teaching and training of medical officers, Consultants, Nurses for Pediatric Emergency. I was also involved in evaluation of Physician documentation, Mortalities and Morbidities in different pediatric emergencies, Strategies development to improve Learning and patient care, advocating child health at national and International Level. I was also involved in improving Quality and Patient safety initiatives in Pediatric emergencies of public sector hospital. As a project lead I am involved in POCUS (Point-of-care ultrasound) its implementation across Emergency Rooms.

Experience in Patient Safety and Quality:

At Child Life Foundation I am involved in multiple Patient Safety and Quality initiatives for improving patient care and quality. I am part of a team that evaluates physician performance in emergency department, compliance towards medical protocols, triage practices, Morbidities, mortalities, Case Reviews and Emergency department Revisits.
