Employment History and Responsibilities:

March 2019 to present I have continued my role as the Director of Nursing services at Signature Healthcare. I travel throughout Kentucky an serve in an Interium Director. I continue to oversee resident care, enforce policy and procedures, conduct in-services, work with physicians as well as, other vendors, residents and their families, OIG and local Ombudsman to ensure resident needs are met. I work with and schedule nurses and CNA's to cover scheduling needs, conduct daily rounds and perform various audits to ensure documentation compliance.

July 2018 to February 2019 (agency) Regional West Garden County Nursing Home Facility through Medical Solutions. As the Director of Nursing, I manage staff through annual performance evaluations, disciplinary actions as appropriate, assist with scheduling, conduct interviews, review referrals, oversee all resident care through care plan meetings, quality and safety meeting, daily rounding and repore with residents and familiy, investigate issues, concerns and grievances, work with state survey teams and local ombudsman. In addition, I network with other department leadership, providers, vendors and community resources to promote a positive image of the facility, unite staff to work as a team and provide a better quality of life for the residents in our care.

January 2017 to June 2017, Stanford Care and Rehabilitation, I continue to serve in the Director of Nursing role. I network with all department directors to include Admissions, Therapy, Social Services, Dietary, Payroll, Maintenance and Housekeeping to ensure all resident health and safety goals are achieved. I monitor falls, skin tears, hospital transfers, medication administration and track narcotics. In addition, I follow residents on antibiotic therapy and type of illness for which the medication is prescribed, manage scheduling for the nurse and cna’s, I conduct in-services on a routine basis, assess learning needs of staff, as well as, gather all care related information for monthly QAPI meetings. I investigate grievances and work through solutions which are acceptable by resident or POA if appropriate, while displaying professionalism and attention to detail.

August 2015 to January 2017, Berea Health and Rehabilitation, I served as the Director of Nursing in LTC. I provided oversight for the facility as a whole in regards to resident outcomes while supervising staff and employee related issues. I assisted in determining skills needed and admission criteria for residents along with the Administrator. I also tracked nursing budget, completed monthly reports, participated in quality assurance and safety meetings, conducted nurse and nurse aide meetings, lead facility meetings, worked with various providers to include medical director, nurse practitioner, psych, vision and dental service vendors to coordinate on site visits.

As the DON, I ensured level of care and oxygen preauthorization were completed in a timely manner, track narcotics and narcotic destruction, falls, skin tears, hospital transfers, employee incident reports and completed workers compensation initial report. In addition, tracked resident and employee illness for Infection Control, completed evaluations for nursing staff and contributed to nurse aide evaluations. I counseled nursing staff as appropriate regarding job performance and attendance while role modeling professionalism and competence.

September 2007 to August 2015, VA Medical Center, experience as an ICU nurse. Responsibilities include proper documentation, skin assessments, starting intravenous fluids, titrating vasoactive drugs, hemodynamic monitoring, suctioning and oral care for ventilator dependent patients, wound care, conscious sedation for procedures, assist with line placement by gathering appropriate supplies and cables needed for monitoring, work collaboratively with other staff routinely and during a code situation, being able to work in a high stress environment and work well under pressure. In addition, serve as charge nurse ensuring adequate staff available to holistically care for our veterans. Also facilitating patient transfers when appropriate and collaboratively work with multidisciplinary teams who are treating patients within the critical care setting.

I have also been working in case management. These duties consist of reviewing consults for appropriateness, call patients regarding preparation for their procedures, perform chart checks and call with results of that test, as well as, send recommendations to the referring physician. I educate patients with newly diagnosed illness/disease and surgery/treatment, set up labs and follow up appointments. In addition, track HCV antibody positive patients in this facility and search chart for viral confirmation to either confirm or delete from the National Clinical Case Registry. I work with all medical specialty services, such as, gastroenterology, pulmonary, cardiology, hematology, oncology, and nephrology.

I have experience performing chart audits. This is to ensure quality measures are addressed upon admission and maintained throughout the patients stay. Key elements in these audits are admission weights, daily skin assessments, pain, assessment part 1 and 2, daily care plan updates and their appropriateness.

I have experience as the VANEEP Scholarship Coordinator. My responsibilities include navigating throughout the VANEEP program by working with all departments to facilitate the best outcomes possible. This is being completed by gathering pertinent documents to be submitted with the participant’s application, working with individuals to ensure application completed correctly, networking with Nursing Education Counsel, Nursing Service, Administration, Fiscal and HTM to facilitate our succession plan for retention. I organized and set up interviews for VANEEP and serve on the interview panel for both VANEEP and NNEI. In addition, I participate in annual reports and follow up on student progress throughout their instruction period.

I assisted in data collection for NDNQI. In particular, data for Dementia Tracers and Prevalence Survey for skin, at Leestown and Cooper Drive. I also participate as a member of the Performance Improvement Team. I have educated new nurses regarding policies dealing with waste and controlled substances, double verification of high risk/high alert medications. Foster a learning atmosphere while educating staff to maneuver throughout the BCMA program to ensure safe and efficient medication administration.

March 2005 to May 2007, Advanced Pain Medicine, interventional pain medicine clinic. Responsibilities included proper documentation, administering medications, starting intravenous fluids; anticipate needs of physician during a variety of procedures, such as, cervical, thoracic and lumbar facet blocks, epidurals, intercostal nerve blocks, sympathetic nerve blocks, and rhizotomy procedures. In addition, recover and discharge patients after their procedures.

January 2004 to February 2005, Central Baptist Hospital, experience as a neonatal nurse. Responsibilities include proper documentation, administering medications, monitoring vital signs, photo therapy, and starting intravenous fluids, as well as being able to work in a high stress environment and care for critical infants.
