PCU nurses also covers short stay observation patients that may stay overnight for procedural immediate intervention e.i. stress test, telemetry monitoring for possible next day ablation, pacemaker placement, cardiac catheterization and post-operative laparoscopic surgeries, Lithotripsy to urethral stent placement to stay overnight for pain control. It has a patient/nurse ratio of 4:1. Other cases handled in this unit are DKA, PE/DVT, GI bleed, alcohol withdrawal, drug overdose, sepsis/SIRS, CKD, ACS, respiratory cases and more.

Clinical Skills Summary – Progressive Care Unit/ Intensive Care Unit
Performs a brief and focused assessment based on admitting complaints upon admission of patient in the unit also including a review of a brief patient’s history and determining risk factors of the condition.
Takes and records patient’s initial vital signs such as blood pressure, pulse rate, respiratory rate, temperature, and oxygen saturation upon admission in the ICU for baseline vital signs in comparison to ER arrival.
Reassesses full comprehensive head to toe assessment including lung sounds, heart sounds, bowel sounds, and reflexes.
Use and interpret Glasgow Coma Scale as a tool for neurological assessment and management and refer for any new unusualities.
Uses the Richmond Agitation-Sedation Scale (RASS) in assessing and managing patient in need of sedation with physician’s orders.
Monitor vital signs using continuous non-invasive vital signs monitor machine, apply cardiac monitor leads, recognize and initiate care for life threatening arrhythmia noted during cardiac monitoring.
Provides safety measures like using physical restraints and chemical restraints with physician’s orders for combative patients and may be at risk for accidental extubation or pulls out any contraptions (FBC, IJ catheter, drainage devices, IV tubing).
Assesses pain using the Numerical Rating Pain Scale for conscious adult patients and Wong-Baker scale for pediatric patients.
Assesses and prepares patient for CAT Scan (plain/ contrast) and MRI or for surgical procedure
Initiate basic life support measure/CPR and ACLS; Assist in intubation; Anticipates and initiates immediate defibrillation of life threatening shockable arrhythmia and pharmacological management of non-shockable arrhythmia.
Function as member of the unit cardiac arrest team/ recognize need for assistance of the hospital Code Blue Team.
Assist with diagnostic and therapeutic procedure e.g., incision and drainage, bedside debridement, bedside intrajugular catheter insertion and Lumbar puncture for CSF studies.
Performs peripheral IV cannulation pediatric and adult patients and administer IV fluids and manage blood and blood products based on the doctor’s order using infusion control devices
Monitor patient’s blood glucose level using a glucometer and initially interprets values to be referred accordingly
Initiates continuous assessment, evaluation of intervention, documentation in nurses’ notes assessment, intervention and management; inform the doctors on the new symptoms, changes in trend of monitoring and development of the condition of patients.
Recognize need for/ assist in endotracheal tube insertion; do suctioning per orem/nares, endotracheal and tracheostomy tube
Performs gastric lavage for nursing management of poisoning/ toxic substance abused patients
Administer nebulizer/inhaler treatments as ordered via ventilator assisted nebulization or through portable nebulizers
Draws ABG sample, and initially interpret its values then refer accordingly for further management for possible intubation, mechanical ventilator adjustments of settings and weaning or possible for extubation.
Inserts NGT and give tube medications/ feedings per Physician’s order
Performs male and female indwelling or straight catheterization.
Administer oxygen inhalation via nasal cannula, face mask, tracheostomy and endotracheal tube and also non-invasive ventilation with doctor’s orders.
Administer blood and blood products; monitor patient’s condition at the initiation, during and post blood transfusion and institute remedial action for any untoward blood transfusion reaction and its process.
Assist patients with emergency referral to other hospital; Prepare documents and ambulance transport for further management.
