Activity Director/ Dementia Coordinator

Development and coordination of activities program designed to promote the residents active involvement with each other, their families and the community, and involve staff in implementing program goals. Plans and implements an activities calendar each month, which includes life skills, education and recreational programming daily. Post calendars throughout the residence also demonstrate documents involvement with all residents, staff, families etc. Provide one to one programming for residents who cannot participate in a group setting; coordinate the transportation of residents to and from events outside of the residence, which may include driving a residence motor vehicle. The Director is accountable for all residents’ presence when leaving and returning to a residence. Taking inventory and purchase activity supplies including recreational equipment, supplies for games, books, current magazines and daily newspaper for living etc. also, operating within the facility’s budget and according to administrative policy. Trains staff on the importance of meaningful activities and other areas regarding the quality of life for residents. Participating in the review of individual Residents’ Service Plans and documents activities progress every six months or when it is a significant change in the resident. Coordinating quarterly special event at the residence, recruiting, training, and supervises volunteers. Also, participates in discharge planning with other member of the management team. Developing the community resources and contacts to enhance the quality of live for residents and being familiar with Adult Care Residents’ Bill of Rights and assist resident with exercising these rights.

Dementia Coordinator

Oversight of all aspects of Activity Program, coordinate Pre-admission assessment/screens, care planning, including identifying behavior triggers, appropriate response and intervention, individual preferences, etc; create and maintain a comfortable and homelike environment, staff and activity scheduling/hiring and firing staff on the unit, oversight of resident eligibility to the program, maintain appropriate residents on the unit. Maintaining all safety requirements for the program and the unit; orientation & training of all employees to work on the light house unit, coordinate discharge process, lead CQI process on the unit, utilize the EMR system and monitor for compliance, maintain resident per policy and procedure, serve as the primary liaison to the community, families and area hospitals, be responsible for ongoing training/learning in the dementia field, train staff in learning efforts to keep all staff training current; maintain effective communication with dietary department to enhance nutrition program for residents and communicate with therapy to enhance quality of care and quality of life issues; coordinate Quarterly family support meetings, meet and exceed all regulatory requirements as established in this state.
