Primary Position--- Director of Nursing

Responsibilities include:

1) Development of wound care program with on-going assessment of all wounds in the building

2) Ensures that optimum resident care is provided by planning, organizing, supervising, and evaluating resident care.

3) Prioritizes duties to ensure optimum resident care is maintained.

4) Demonstrates knowledge of, and directs Nursing Staff in accordance with local, state, federal rules, and regulations.

5) Conducts Investigations, develops and implements corrective action using unplanned occurrence tool/format.

6) Ensures adequate staffing ratios are in place for all shifts.

7) Completes staff evaluations in timely manner and ensures all staff evaluations are completed according to departmental procedures.

8) Mentors Clinical Care Coordinators/Unit Managers and floor nurses

9) Provides educational in-services, formal and informal training sessions for staff.

10) Assures resident care is documented accurately in medical record, and Comprehensive Care Plan.

11) Assists with the plan of correction (POC) for State of Michigan annual and complaint surveys

12) Interviews and hires staff. Evaluates job performance and working relationships of staff. Resolves personnel problems utilizing management policies and procedures and administers disciplinary action appropriately.

13) Monitors Quality Measures (QM's) in building for short stay and long stay thru both point right data system and CMS Casper reports.

14) Attendance in Quality Assurance and development of PIPs for QM areas that are > the 75th percentile

15) Assists with daily and weekly reporting to NHSN/CDC for resident/staff vaccination status to include both Flu & COVID-19 vaccines. Ensures pathway data reporting and Point of Care (POC) testing results of residents, staff, visitors, and/or vendors are entered into NHSN/CDC in a timely manner.

16) Ensures the Infection Control line listing is completed monthly and properly mapped throughout the facility. Makes sure antibiotic stewardship is closely monitored with doctor follow-up on all infections that do not meet McGeer’s criteria.

17) Completes and tracks Incident and Accident reports for falls, skin tears, bruises, pressure ulcers, resident-to-resident occurrences, etc.

18) Manages the re-hospitalizations and creates weekly reports thru QI reviews in Point Click Care (PCC)

Clinical Resource Manager/MDS Coordinator

Responsibilities include:

1) Know, understand, incorporate, and demonstrate the Mission, Vision, and values of the organization thru behaviors, practices, and decisions.

2) Evaluate, develop, mentor, coach, counsel, and discipline department staff… supporting community personnel from other departments through coaching and mentoring to help achieve optimum standard of excellence.

3) Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of CAAs & triggers.

4) Conduct the interview of each resident for the resident assessment… evaluating each resident’s condition & pertinent medical data to determine any need for special assessment activities.

5) Coordinate & oversee the entire RAI process including completion of the documentation as necessary to ensure that time frames are met by the interdisciplinary team.

6) Coordinate the development of a written plan of care (preliminary & comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.

7) Collaborates with interdisciplinary team members to coordinate plan of care with discharge plans. Attends care conferences to review these plans with resident and family members.

8) Ensure that an initial Resident assessment is completed within appropriate time frame of the resident’s admission and locked and electronically submitted in a timely fashion.

9) Provide reports (weekly RUG utilization rate report, Facility Quality Measure report, Five-Star Report, missing assessment report hen needed to the Administrator, Director of Nursing, & other members of the interdisciplinary team

10) Attend bi-monthly meetings with administrator, Director of Nursing, & Nurse Managers to discuss facility Quality Measures and how they affect the facility Five-Star rating.

11) Attend monthly Mission Driven Quality Improvement (MDQI) meeting with medical director & all department heads to present and discuss action plans based on department performance… in the MDS department, the action plans primarily dealt with how to maximize ADL reimbursement

12) Maintain a working knowledge of applicable Federal, State, and local laws & regulations, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, & professional behavior

Secondary Position--- MDS Nurse

Responsibilities include:

1) Assessment of patient’s medical and physical condition upon admission and throughout their stay at the facility.

2) Collection of patient data sourced from a variety of individuals ranging from physicians, other nurses, nursing aides, therapists, and the patient’s family members and using this data to analyze the patient’s post-discharge needs.

3) Regular review of patient charts including progress notes from all disciplines in preparing the patient’s care plan.

4) Gather & analyze the patient information so to maximize the patient’s insurance benefits within the scope of the Medicare regulations and beneficiary benefits.

5) Utilize the data gathered as a guide in determining the reimbursement rate for the individual patient.

Midnight Supervisor (fill-in) & Admissions Nurse

Responsibilities for Admission Nurse include:

1) Completing a full, comprehensive physical exam and assessment of a new admission.

2) Creating a detailed skin assessment of the patient focusing on problem areas (pressure ulcers, incisions, hematomas, skin tears, etc.) that may require further nursing measures

3) Developing a plan of care most appropriate to meet the specific patient’s needs

4) Coordinating admission orders from the hospital to the SNF physician.

5) Transcribing all approved orders to the appropriate medical records.

6) Assessing the new patient for a possible falls risk and initiate appropriate nursing measures.

7) Orientating the patient and family to the room & facility so they feel welcome.

8) Educating the nursing staff about the new patient’s medical and nursing needs.
