We are pleased to present the following offer of employment for you to join Hospice of the South Coast, a part of Care Hospice. This letter and accompanying enclosure will summarize important details of matters pertaining to your employment. Also enclosed is information about current benefits, which are provided to our full-time colleagues Here are the specifics of our offer:

o Position: Registered Nurse Case Manager

o Status: Full Time

o Proposed Start Date: December 27, 2022

o Base Compensation: $55.00/Hourly

o Supervisor: April Moyer

As part of the total compensation plan, Care Hospice comprehensive benefits package includes the following:

o Health, Dental and Vision Insurance - Coverage begins on the first of the month after the completion of thirty (30) days of continuous service. o Long Term Disability/Short Term Disability - You will be covered in accordance with the terms of the elected Plans.

o Paid Time Off – You will accrue PTO each pay period, up to 20 days or 160 hours in your first year.

o 401(k) Plan – You will be eligible for participation in accordance with the terms of the Plans.

o Mileage reimbursement & cell phone stipends for eligible colleagues per company policy.

Your offer of employment is contingent upon you fulfilling each of the following terms: o In accordance with our Drug Free Workplace policy, all prospective employees are required to be drug-tested. o Acknowledgement of Company Handbook and Confidentiality Agreement: As a Care Hospice employee, you are required to acknowledge that you have read the Care Hospice employee handbook.

On or near your first day, you will participate in a new colleague orientation where we will provide a company and benefits overview.

This offer letter forms the complete and exclusive statement of your employment with Hospice of the South Coast, a part of Care Hospice. It supersedes any other agreements or promises made to you by anyone, whether oral or written. Changes to the terms of this letter require a written modification signed by an authorized employee of Hospice of the South Coast, a part of Care Hospice.

Additionally, Hospice of the South Coast, a part of Care Hospice, reserves the right to revoke this offer should we not receive a satisfactory reference check, background and/or drug screen for you. These forms must be completed and submitted prior to your first day of employment.
