Collaborate with patient’s providers to overcome barriers to improve STARs rating and review KPI to identify service gaps and opportunities like quality and health literacy metrics. Consult with physicians to optimize medication therapy and counsel patients regarding medication dosage, drug interactions/contraindications, generics, less expensive options, over-the-counter (OTC) products, medical equipment, disease management and identify and address member specific medication adherence barriers. Research, prepare, and dispense medications and other pharmaceutical products, including dispensing and compounding prescriptions. Follow-up with medical providers to clarify prescribed medications, dosages, refills, interactions, and allergies; suggest alternative medications and answer medical provider’s questions. Maintain accurate patient records, review and monitor prescription claims data and prescribing practice to ensure quality assurance. Accurately process insurance claims to resolve member’s issues and prevent payment rejections and participate in 3rd party audit.
