I have been a Registered Nurse for over 35 years. My work history varies according to the times in which I worked. When I got my Associates degree in 1986, a graduate could be a DON at a large hospital back then. Most of my history is in Arizona and unfortunately background checks are rarely available as facilities are bought and sold there like houses. I retired for a time due to health concerns and moved back to California only to discover I had been misdiagnosed. I returned to work in 2020 and have endured prejudice due to being Caucasian and a senior. I love nursing and am dependable, hardworking, and loyal. (I am fully vaccinated and have never had a positive Covid 19 test result.)

Southland Care Center RN Supervisor

11701 Studebaker Road 8/6/21-6/1/22

Norwalk, CA 90650 Shony DON

This is a skilled nursing facility that maintains a 90-95% capacity with heavy turnover. I started on a split shift of 3 days and 2 nights. When the full-time night supervisor left, I was asked to take this position. It required IV management, admissions, and discharges. Extensive PCC (Point Click Care) use. Consistent transfers to acute care settings as the residents were newly admitted from that setting. I loved this job and the employees I worked with but want to get a higher education. I am unable to work full-time and go to school full-time and get the grades I want.

Los Angeles, CA 90037 Stephanie Onwaser, DON

Hired as a weekend supervisor and side promoted to full-time on 4/24. Facility was LTC with psychiatric focus. I worked extremely hard to make the place a little better and was laid off because of management's prejudice.

Royal Care Skilled Nursing RN Supervisor

2725 Pacific Ave. 6/17/20-9/11/20

Long Beach, CA 90806 Carol Dollarhide, ADON

A skilled nursing facility which strictly adhered to the Covid system of separation of resident’s and new patients. I hired in for the evening shift on Saturday and Sunday. I managed IV’s, extensive admissions, and assisted the nursing staff as needed. Especially, extensive conversations with family due to no visitation at the time. I wanted part-time work and was laid off because I would not, consistently, com into work during the week.

Mingus Center Charge Nurse, Days

636 N. Main Street 3/2009-4/2014

Cottonwood, AZ 86326

928-***-****

This was a non-voluntary psychiatric and drug rehabilitation center. Most patients were admitted in full body chains escorted by police. I did all the jobs as it was a small facility. I also attended a daily Team Meeting to discuss patients and their program. For 6 months I covered the DON position during a change over. The facility was sold, and the new owners only kept the new DON.

Elite Healthcare Staffing Staff Nurse

7200 W. Bell Road, Ste. H107 3/2004-2/2008

Glendale, AZ 85308

As a Registry nurse I worked all over the Phoenix Valley area in a variety of jobs from Medical only units to ICU (Intensive Care Units) and Emergency Departments. I practice everything from blood transfusions to codes. I decided to find steady work and get out of the heat of the valley.

Phoenix Memorial Hospital Staff Nurse

1201 S. 7th Ave. 3/2001-2/2004

Phoenix, AZ 85007

602-***-****

This was a hospital that had been bought by a group of doctors and was newly opened. I worked in the med/surg unit. I was responsible for post operative, preoperative, admission examinations, blood transfusions, IV starts and management, sterile techniques, dressings, and detailed order review of all charts being night shift staff. I left for better pay and better working conditions.

Previous positions include dialysis care, utilization review, psych units with both adults and children, always in charge or supervisor positions. I have learned patient and staff management by the process of experience.

I attended Yavapai College in Prescott, AZ from 8/1983-5/1986 and graduated with an associate degree in nursing. I was licensed in 1986 and transferred it to California on 2/1/2019.

My license is good until November 30, 2024.

I believe in continuing education and took many CEU courses in Arizona even though the Arizona State Board of Nursing does not require them for relicensure. A list of all the CEU courses I have attended through the years can be made available if desired.

I have a current BLS for healthcare workers that expires on September 2022.
