5. JTTC Joint Venture Contract E34026. Replacement of 11 Hydraulic Elevators throughout New York City Transit Authority system.

6. MLJ Contract A35302/37116. Grand Central/Time Square Shuttle Stations. Total rehabilitation of train stations and its electrical systems.

7. Scalamadra/Gramecy Joint Venture. Elevator installation and water main work on street level. 8. RSMK Contract P36476 Rehabilitation of Circuit Breaker House. Worked as a Site Safety Engineer for New York City Transit Authority.

Safety Supervisory February 2016 – June 2018:

Since November 2017 to present I have been approved on numerous NY City Transit Authority/Long Island Railroad/Metro North projects as a Site Safety Supervisor. Four contracts that I have been approved as a CSS and worked on them as follows:

Site Safety Supervisor:

1. New York City Transit S/48005 Communication Based Train Control (CBTC) (Comstock Company) 2. New York City Transit S/32176 New Kings Highway Signal Relay Room located above elevated train tracks. (Comstock Company)

3. New York City Transit S/48009 Emergency Signal Upgrade (Comstock Company) 4. New York City Transit P/36698 Elevator Installation 57th and 7th Street (Forte) Approved as Safety Engineer On New York City Transit Authority Contract A/37628 Sandy Resiliency, Emergency Booth Communication System (Voyce Technology) 2

Project Span Service March 2016 – June 2018

Brooklyn, NY

Two documented contracts with LIRR

1. Contract MTACC LIRR, CS- 084, C08. Substation site 43rd Street. 37th Avenue. EJ Electric. Five year contract, April 2016 - April 2021. Wiring of transformers and switch gear, conduit work. 2. Ensuring all DOT Permits is correct and guidelines followed. That all lifts and mechanical equipment is inspected daily while in use.

3. Maintaining that the safe work plan is in force. 4. Contract LIRR project #6158. Rail works. Atlantic Branch Half tie replacement, March 2016 - November 2016:

Removal and replacement of wood ties with new composite half tie. Using modified high rail excavations, jack hammers and other mechanical equipment.

Perform site safety orientation for all employees, which include review of company safety policy and safety, health and environmental control plan.

Ensuring all high rail equipment is inspected daily and compliant to all specs. Monitoring all work groups ensuring PPE complaint and working by the Safe Work Plan.

Ensure all employees know and understand the safety rules and requirements for the project and have a written copy of such rules.

Issuing all required personal protected equipment to employees and ensuring the PPE is properly used.

Perform fitness for duty on employees before the start of every tour and monitor for compliance during the work day.

Perform tool box critique and maintain a daily safety log.

Maintain and update all site permits and OSHA 300 log, dot permits.

Investigate and submit reports on all accidents, showing the cause and recommending future correct measurements, then ensuring the implantation of the corrective action plan.

Perform safety inspection of work site; submit report of finding and any hazardous areas. Submit recommendations to project manager to mitigate the hazard.

Attend weekly safety Project Managers, LIRR meetings pertaining to work performed and future work schedule.

Constantly monitor work area to ensure The Safe Work Plan is in force.

Inspect work area at end of tour making sure no hazardous of unsafe condition excite to prevent the safe passage of trains.

Judlau Contracting, Inc./OHL Group, NYC October 2016 – July 2017 Safety Engineer Supervisor

Oversee safe operations of Judlau TC Electric and Sub-contractors employees ensuring that the safe work plan and Safe, Health & Environmental Control Plan is in force. Job: Metro-North Railroad Hudson Line Power/C&S Improvements CP-5-CP35 – Contract #100*******

Excavate and install concrete and PVC conduit, build substations, install new power lines damaged by Super Storm Sandy. This all under adjacent high speed rail service and energized third rails.

Protecting the workers and ensuring they are always compliant with OSHA, FRA, Metro-North and Judlau safety Rules and Regulations.

New York City Transit Authority, Brooklyn N.Y. July 2011 - October 2015 Train Supervisor/Safety Supervisor

Two documented contracts I was a Train Service Supervisor/ safety supervisor during my employment with the transit Authority:

1. Contract NYCT # C/521 I0/14th Street/usq. Barbaro Electric. Two year contract March 2014 - October 2015. Installation of conduit and security cameras. 3

2. Contract NYCT# S/32761151st. TC Electric Two year contract, July 2011- April 2014. Removal and replacement of signals and all components with the signal system. J-Track Replacement of rails and ties, both concrete and wood.

Responsibilities for the above two contracts were:

Perform Daily pre job safety meeting for construction flaggers on rule of the day, and safe work practices.

On arrival to work site assess the scope of the work to be performed and any hazards. Critique Transit and contractor employees on the issues.

Perform fitness for duty and PPE requirements for contractor employees.

Check and verify all contractor employees have up to date training for their job function and proof in their possession.

Inspect contractor machines and equipment that it is authorized to be used on Transit property and that they are free of defects. If found immediately removed from service [tag out lock out]

Ensuring that the proper flagging configuration is set for the safe passage of trains and employee contactor safety.

Constantly monitor contactor employees on work being performed and stop or correct any unsafe work being performed.

Ensure that job site environmental, health and safety audits are performed.

Monitor to ensure the health and safety plan and SDS are enforced.

Investigate and write reports on all accidents and corrected measures to prevent future occurrences.

Inspect work area at the end of shift ensuring no unsafe or hazardous conditions exist.

Prepare written report at end of every tour concerning work performed and report any deficiencies, and corrective actions to be taken.

Attend weekly contractor safety meeting on work previously done and future work to ensure the safe work plan is followed.

ADDITIONAL EXPERIENCES

For all contracts, I supervised multiple General Orders [G.O.] that were associated with these jobs, ensuring safe implementation and maintaining compliance of the G.O. throughout its duration; performed pre- job safety meetings [Toolbox meetings] both in crew quarters and in the field [fitness for duty] "PPE enforcement", review rules and regulations and safety concerns of the employees and review any G.O., monitored work being performed and ensured safe work practices were adhered to; held bi-weekly safety meetings concerning the Accident Prevention Plan [APP] and ensured all safety issues were addressed and corrected if needed. Train Service Supervisor in Construction Flagging 2011-2015

Check contractor equipment that no defects and in good working order and no unauthorized equipment is being used.

Verify all contractor permits are current and in force.

Investigate all accidents and submit reports on cause and corrective action to prevent future accidents.

Constantly monitor work area to ensure that the safe work and accident prevention plan are in force.

Inspect work area at end tour to ensure no unsafe or hazardous conditions exist. Train Service Supervisor Work Train 2009-2010

Perform daily pre-job safety meeting for work train operators on the rule of the day, and safe work practices.

Perform visual inspection of construction material on work train verifying equipment is properly secured from fall off while in transit

Ensuring no unauthorized equipment is being transported, ex. [gasoline]

Making sure proper placement of work train/trains in work area, [G.O] general order areas. 4
