Oversee and coordinate with community mental health & substance abuse treatment services, coordinate managed care requirement as per DOMH, HRA, OPWDD regulations for 25 clients.

Provide counseling, implement crisis intervention, and de-escalation techniques when necessary.

Conduct psychoeducation groups on topics such as discharge planning, stress management and the health risks pertaining to substance abuse.

Develop and maintain individualized treatment plans, 400 monthly follow up notes for 25 clients, and documenting clients’ progress in accordance with DOHMH and DHS guidelines.

Find appropriate housing opportunities and execute appropriate discharge plans for clients.
