Founder, owner, and solo Occupational therapist of unique therapy services which incorporates equine movements in therapy sessions to facilitate neurological and physical influences of patients seen to achieve individualized goals. PATH Certified International Therapist, Certification in AHCB and have furthered education and necessary requirements for higher education for HPCS Credentials, in which only 80 or fewer therapists have worldwide. Markets and networks for private practice. Performs initial evaluations, re-evaluations, letters of medical necessity, progress reports and discharges for children in need of Occupational Therapy intervention. Develops individualized plan of care, incorporates caregiver education and training during therapy session for carry over of learning experiences and carries through treatment. Performs for patients/clients in homes, school settings and Therapeutic Day School settings environment assessments for safety, functional abilities and limitations based on diagnosis. Provides education to patients and caregivers/family, trains patient skills in BADLs/IADLs and functional mobility to become as independent as possible in the least restrictive environment with or without the use of adaptive equipment based on Doctor's orders for required precautions. Discusses with the rehab team of nurses, other therapists, case managers and social workers patient’s progress, safety, and ability to perform functional tasks safely. Not limited to home and environmental evaluations in the home setting, but also experienced in Total Shoulder Replacement and Rotator Cuff Repair therapies, Autism, Developmental Delay, Total hip and knee Replacements, dementia, and general weakness, just to name a few diagnoses.

RehabCare, Cedar Crest Health Center, Janesville, WI January 2008-2012

Director of therapy services for Cedar Crest until May 2012. Census includes 100 Independent units, 86 skilled

nursing beds and 46 assisted Living units. Manages and establishes with MDS coordinator RUG levels and ARD dates for Med A census. Communicates with nursing, ADON, and DON concerning patients and concerns. Incorporates SMART system for RUG levels, schedules, and ARD dates. Mentors/manages team of 1 PT, 2 PTAs and 2 COTAs to be successful and to provide the best quality of care for patients and for our customer while maintaining proper productivity and not only meeting but exceeding the expected budgets. Uses SMART system on iTouches for documentation.
