• Educate communities and individuals on their wellness options and pathways.

• Coordinate with internal and external wellness stakeholders to improve program efficiency.

• Follow assessment procedures for ill health and care management.

• Manage multiple care and support cases for children and elderly to meet them at the point of their needs.

• Source and access appropriate treatment and care interventions using the most effective method of funding or insurance applicable for the medical condition.

• Mobilized and sourced over $200k worth of cash and gift items to support Covid 19 communities in 2020 to 2021
