• Preparation of HIPAA authorizations in order to request plaintiff’s medical records for investigation of legal matter;

• Review medical records and prepare summaries to insurance carriers regarding specific issues relevant to plaintiff’s medical issues to be used at depositions, arbitrations, mediations and trial.

• Created medical record tracker charts which include: 1. Provider name and address, phone number and fax number; 2. Date request was sent either by mail or fax; 3. What category of records are being requested (i.e. complete chart, entire record or more specific time frame); 4.Notes section to include all contact info of who I spoke to and on what date and status of request; 5.Date records received;

• Follow up phone calls after 10-15 days to determine status of request to confirm request was received, if sent by regular mail. Follow up phone call same day of request, if request faxed to provider or forwarded to outside copy vendor;

• Once records are received they are scanned and saved to network and hard copy is placed in labelled folder for paper file. Folders are given to attorneys for their review. I would review records for additional providers in order to request records;

• Review Notices of Intent provided by Defendants and request records listed if not previously requested

• Schedule independent medical examinations with doctors and prepared records for the doctors’ review;
