WORK EXPERIENCE:

Data Analyst and Case Manager (Administration, Compliance Unit)

Kirkbride Center, Philadelphia Pa. (Hospital and Rehabilitation) - Philadelphia, PA November 2021 to Present

I manage over 600 patient’s information documents and using software tools such as Redcap, Credible, Methasoft, Weblims for urine and serum labs, HCS-eMR, and soon PhilaVax.

I maintain spreadsheets of documents from patients’ charts, tracking documentation on addiction treatment and recovery of patients from admissions to discharge and updating same as required.

I audit various documents per patient used in critical points of care products and services offered to over 600 hundred ( 600) patients through the Center of Excellence (COE) program.

I am responsible for test running a newly developed interactive Behavioral and Health Management information system for the Center of Excellence Database.

I am a part of a team integrating client chart information or data to interact with Redcap in collaboration with the I.T. team, and using the new information management system to enable better uploading, updating, storing and improved reporting of the Center of Excellence program along patient document core data points.

I identify limitations of the new interactive management system on a progressive basis, including timely communication to the Program Director and the information technology team about possible improvements as gaps are detected in the new system’s functionality become obvious.

I manage care documents and enter updates in Redcap on points of continuing care and referrals services. Some of these documents include as Philadelphia fight Community Health Center for HCV (hepatitis C virus) or Ob/Gyn visits, other kinds of Doctor visits, BARC-10, Patient Choice Forms, Narcan use training sheet, Referral forms listing services selected by patients to enable transition from addiction to better post-discharge outcomes, Prescription Drug Monitoring Program (PDMP), Psycho Social Assessment (PSA), Laboratory urine tests at time of admission, and Serum labs. Other documents are Individual notes, History and Physical (H and P), and doctor’s notes.

I ensure monthly follow-up contact made to discharged patients by call, email or letter. I also do field assignments as part of reaching discharged patients.

I track the Covid Log daily from the nursing station and admissions which is reconciled with data from Credible (Web-based EHR for mental/behavioral health) and is tabulated, updated, and submitted on an excel spreadsheet to the to the DOH.

I meet with patients – new and returning on a daily basis, follow up with Medical Records, and update hospital admissions, nursing stations, and therapists on gaps in patient chart completion.

I track compliance of patients’ suicidal ideation and homicidal ideation check-off on each patient’s Psychiatric Admission Note and Nursing Assessment document and updating the relevant spreadsheet.

I achieve at least eighty percent (80%) patience satisfaction survey results for the Compliance unit monthly; surveys are taken from each floor unit of patient recovery at the 3.5 H-Clinically Managed Highest Intensity (level of care referred) facility – clients assessed are in the COE program from methadone inducted patients in detox to discharge.

I receive incident reports on patients leaving without routine discharge or AMA, AWOL, etc. for Compliance unit.

I ensure HIPPA compliance in discharge of duties.

Medical Records Assistant - Merakey - Philadelphia, PA January 2020 to November 2021

I managed patient chart projects covering Philadelphia, Montgomery, Delaware, and Bucks counties.

I prepared patients charts for claims disability benefits eligibility receipts and mailed prepared documents back to requesting legally certified persons or organizations, including faxing relevant parts or pages of patient charts to insurance companies to process such claims.

I updated spreadsheets, entering client data or information into a comprehensive content management system (CMS).
