EXPERIENCE:

Physician: Concentra, Tacoma

September 2022 to present

Presently serving as the center medical director seeing patients 95% of full time with 5% administrative time for oversight of the medical aspects of clinic operations. Evaluating and treating primarily occupational medicine cases with 3 or 4 urgent care cases mixed in throughout the day. Typical daily daily census ranges from 125-150 patients a day including physicals, new injuries, injury follow ups, drug screens, visits for TB clearance, immunizations.

Physician: Marque Urgent Care

Marque Urgent Care (was Partners Urgent Care until purchased in 2/2018), San Diego County, July 2011–August 2022, part-time.

Evaluating and treating patients in free-standing urgent care settings located throughout Orange, Riverside, Los Angeles and San Diego Counties. Patients’ ages range from one month to geriatric. Problems treated are of an acute nature with the urgent care serving as back-up for Sharp Community Medical Group primary care physicians. High acuity/complexity cases are stabilized and transported to an appropriate ER. Each clinic also treats work comp patients as well. Typical flow rates average 3-4.5 patients per hour with single physician coverage assisted by an MA. No licensed nursing staff on site.

CoVid-19 pandemic response: Marque Medical provides telemedicine services in a thoughtful, professional, clinically rigorous and HIPAA-compliant manner to patients throughout all of Southern California. I am a provider on their telemedicine platform and am currently evaluating patients with possible CoVid-19 infections and as well as other acute complaints from both my home and at Marque’s physical locations whenI am schedule for in-clinic shifts.

Physician: San Diego State University, Student Health Services, San Diego, CA, July 2000 – 4/2020 (retired)

Lead physician, Special Projects: January 2019 to January 2020. In addition to the full-time clinical responsibilities listed below in the next paragraph, this assignment involved the establishment of a walk-in triage area with both RN and provider support for injured and ill students. This assignment included the supervision of 6 RNs who were engaged in face to face and phone triage as well as providing direct patient care for simple problems under protocol such as uncomplicated UTIs, sore throats URIs and STI testing in asymptomatic individuals. Other projects assigned to me included: evaluation of the Student Health Service phone system to improve access for students; development of an in-house prescription medicine inventory system on our current EHR: participation in text message set-up and implementation; coordination of staff measles immunization testing and immunization, coordination of CPR recertification for staff; coordination of mask fit testing for staff.

Appointed Care: September 2013- April 2020, full time. Due to budgetary constraints within Student Health Services, the Urgent Care was closed and those of us working there were re-assigned to Appointed Care which was a blend of lower acuity urgent care-type services and typical Primary Care services, all of which were on an appointment-only basis. Typical cases included: acute wound care (some suturing), fracture care (splinting), minor trauma care including post-concussion evaluation and follow-up, gynecologic, cardiac, dermatologic, gastrointestinal, neurologic, pulmonary, surgical, ophthalmic, psychiatric issues (anxiety, depression, drug and alcohol abuse counseling) and occupational and sports-related injuries, preventive exams such as performing pap smears and STD screening, contraceptive management, interpretation of x-rays and EKG’s; supervision of mid- level providers. Also assisted RN’s in Triage with complex and/or high acuity patient evaluations many of which are referred to a higher level of care and those requiring ambulance transport were stabilized and monitored until transfer to the care of 911 responders was completed. Urgent Care: January 2008 to August 2013: had a census ranging from 70-120 patients

(15 to 50 years of age) per 9-hour day. Typical cases included: acute wound care

(suturing), fracture care (splinting), minor trauma care: gynecologic, cardiac, dermatologic, gastrointestinal, neurologic, pulmonary, surgical, ophthalmic, psychiatric issues (anxiety, depression, drug and alcohol counseling) and occupational and sports- related injuries; interpretation of x-rays and EKG’s; supervision of mid- level providers. Patients often presented with acute complex medical problems requiring extended observation & IV/respiratory therapies. Patients requiring emergent care on presentation were stabilized for transport.

Lead Physician/Urgent Care: July 2000 – December 2007: Oversaw daily operations of UC including provider & nursing staffing levels, and patient flow issues; delivered direct patient care and consultative services to physicians and mid-level providers on patient management and disposition; developed a per diem provider pool including recruitment, training, scheduling and evaluation of per diem staff; reviewed all provider requests for planned absences from both UC and Appointed Care areas and scheduled coverage as required; completed performance evaluations for UC-based providers and lead nurse; attended weekly building-wide supervisory meetings; assisted the Medical Director with projects as required; acted as the Medical Director’s agent in his absence. Chair, Therapeutics and Safety Committee, July 2004 - November 2009. The committee was tasked with evaluating requests for new equipment and pharmaceuticals to be used within SHS in terms of need, impact on current practices, cost of acquisition and maintenance, and when relevant, cost-effectiveness. The committee’s recommendations were then forwarded to the SHS management team for review and either approval or rejection. The committee also organized and coordinated SHS’ participation in semi-annual disaster preparedness drills for both campus and city-wide events. Several drills included coordination with off-campus federal and state agencies such as the San Diego County Haz-Mat Team, Homeland Security and the CHP. The committee was also responsible for all aspects of Code Blue response within SHS including staff training and ACLS certification, equipment and medication acquisition and maintenance,

Other current professional activities:

Member, Peer Review Committee, HealthNet Federal Services, January 2018 – present; United Health Care, July 2014- December 31, 2017. Member of HealthNet’s Peer Review Committee.The committee meets monthly to review cases that have been identified as having patient safety issues in the course of delivery of care to the beneficiaries of the Tri-Care West region of the US and make recommendations as to what the involved providers and facilities might do to prevent similar occurrences in the future. (United Health Care managed the Tri-Care contracts with providers and facilities in the western region until 12/31/2017. As of January 1, 2018, HealthNet assumed the contract management)

Member, Credentialing Committee, HealthNet Federal Services, January 2018- present. The Credentialing Committee meets monthly to review individual provider and facility applications for entry into the Healthnet Tri-Care network. These applications are brought to committee because they pass the threshold for the maximum number of settled lawsuits against them without requiring committee review. The committee accepts or rejects the applications based on a review of the types of suits that were settled, the reason for the suits, the dollar amount of each suit, and the dates/timing of them. Other factors also are considered such as geographic location and availability of provider or facility specialty services in the immediate area of the applicant. Prior Work History:

See patients in the Level II portion of the Emergency Department ranging in age from one month to geriatric. Cases include minor trauma as well as a full array of medical and surgical problems and complaints. Patients were evaluated and treated, with some requiring extended stays (6 to 18 hours) prior to admission or discharge. Admissions from the Level II were common. Also perform medical clearances for acute psychiatric patients prior to transfer to an inpatient facility and for recently arrested prisoners being taken to jail by the police.
