Provided multidisciplinary team leadership, including medical director, nursing, pharmacist, mental health, and rehabilitation team. Focus on end of life, palliative, and hospice care.

Led team turnaround addressing low employee morale, engagement, and productivity and steadily dropping patient census. Through leadership coaching, change management, and communications able to retain 90% of existing employees and increase productivity by 10%

Lead budgeting, forecasting, and financial analysis for a clinical site of over 40 employees with an annual budget of 10.8 million. Organized and implemented patient-centered care conferences with the medical home team. Care conferences included PCP, pharmacist, psychologist, case manager, and patients. Through motivation interviewing, Identified the patients’ goals and barriers to care and increased engagement in their medical care, increasing compliance and patient satisfaction.

Partnered with the interdisciplinary team to engage the patient in the appropriate level of care. Based on cognitive and physical ability and disease process, this may mean moving from independent living to assisted living or palliative care to hospice.
