I am a medical graduate with a specialization in Emergency Medicine and have relevant experience for the post. Emergency care, pre-hospital care, disaster management planning, and its training were started in India in 2009 with Indo- US program a Masters in Emergency Medicine(MEM), grown over the period of time, and many of the graduates trained under this system. My training in Emergency Medicine (MEM) was completed in 2011. In 2011, after EM specialization, I got the opportunity to start and supervise a similar MEM program as a Course Director in TMH, Durgapur, West Bengal, India, The MEM curriculum was designed in the USA and it was based on their EM curriculum of the residency training program. The clinical training of MEM was based on the same curriculum, which includes didactics for 9 hours/ week by USA and local faculties, bedside teaching, Triaging and ED evaluation of cases, rotations in other specialties, workshops, mortality meet, procedural training, ultrasound training for diagnosis as well as procedure, pre-hospital care training, disaster management training, Resuscitation (ACLS certification of AHA), Trauma care, etc. Initially, I was trained and supervised by the USA trained Emergency physicians during my MEM residency program at Max Hospital, Delhi, India in affiliation with George Washington University, Washington, USA, and similar training was carried forward to The Mission Hospital, Durgapur, India in affiliation with North Shore Long Island Healthcare, New York, USA as a course director and started supervising the residents and attending consultant for the period of 4 years, which has grown over the period of time and the department achieved 27 MEM training residents of different years, 4 attending consultants in Emergency Department. During these 4 years, more than 10 research and pilot studies were done under my supervision, out of that 3 studies have won awards in EMCON and EMS Asia 2014, and 7 were published in international journals.

In 2013, the US government's top EM body, the American College of Emergency Medicine (ACEP) recognized my significant contribution to Indian EM and awarded me a US scholarship in 2013.

In 2015, the Royal College of Emergency Medicine (RCEM) considered my credential for their MRCEM examination as one of the examiners. The Ethics and values in medical practice came with the practice of Evidence-based Medicine, which mentored me in such a way so that there shouldn't be any harm to the patients and do good with respect.

EMERGENCY MEDICINE DEVELOPMENT IN JHARKHAND:

The Medica Superspeciality Hospital, Ranchi is 250 beds Multi-Specialties hospital with seven beds Emergency Department. In 2015, this was one of the Emergency departments which started practicing Emergency Medicine in the Jharkhand state. There was a lack of ED processes and standard operating procedures in the department, which we completed in the first six months of my joining.

The changes in infrastructure were required to practice Triaging, Resuscitation, conscious sedation, etc. in Emergency Department, ED designing was done and changed as per requirements. Gradually the people of Jharkhand started recognizing the Emergency Department of Medica Hospital as their preferred choice during any Emergencies. The Health Point Hospital, Ranchi is 150 beds Multi-Specialties hospital. The Accident and Emergency department has a turnover of 45-50 patients per day and is equipped with a bedside noninvasive monitoring system, crash cart, resuscitation suite, and minor operating theatre. Doing all the resuscitation / triaging independently. We often see mixed cases like pediatrics, Orthopedic, Surgical, Cardiac, Medical, Trauma, Neuro Surgery, Neurology, and Obs. Gynecology, ENT, Eye Emergencies, etc.

The Pulse Superspeciality Hospital, Ranchi is 150 beds Multi-Specialties hospital. The Emergency Department has one-bed Triage, a one-bed Resuscitation suite, and 5 acute care beds.

We developed the team to manage all Emergencies 24/7 with the evidence-based practice of Emergency Medicine.

The process of Emergency Department & Pre-Hospital patient Care workflow has been documented and implemented.

The checklist has been developed to monitor error-free functioning and maintain good compliance with the implemented ED processes.

The Emergency Clinical care delivery is being monitored and the scope of improvement in current practice has been prioritized with continuous Emergency care training INTERNATIONAL ACADEMIC EMERGENCY MEDICINE EXEPERIENCE: THE MISSION HOSPITAL DURGAPUR, WEST BENGAL.

The International Academic Emergency Medicine started in India in year 2006 and since then I am part of it in different role right from Chief Resident to Program Director of Masters in Emergency Medicine. I got the right opportunity to practice International Emergency Medicine in the INDO- US program from North Shore LIJ and Hofstra University, New York, USA, started this program in The Mission Hospital, Durgapur, India. This MEM program is one of the EM program in India which was in the Rural Part and successfully running for last 5 years in affiliation with North Shore Long Island Jewish Healthcare of New York, USA.

Apart from running the 3rd batch of residency with 10-12 residents per year at The Mission Hospital, I was actively involved in the clinical part of Emergency Department . The total strength of the Emergency department was, one Senior Consultant,3 Attending Consultant, 28 Residents of Masters in Emergency Medicine in three batches.

The academic commitments were, class room teaching 9 hrs per week, Bed side teaching for the better patient evaluation, USG guided procedures and diagnostic, Grand rounds, Mortality meet, Audits etc.

All the International guidelines and protocols has been implemented and followed in the department. All the residents prepared the Log Book of the patients they see in ED and are involved in research activity. At present, more than 10 research and pilot studies were done under my supervision, out of that 3 studies has won awards in EMCON and EMS Asia 2014. The Mission Hospital has got NABH accreditation in year 2013 and the Emergency Department has done exceptionally well and maintained the standard. In year 2013, The American College of Emergency Physician(ACEP), USA has recognized the outstanding growth of Emergency Medicine in rural part of India and granted a Scholarship to attend their scientific conference.

Clinical experience in Accident and Emergency in Max Hospital, Saket, New Delhi, India The Accident and Emergency department of Max hospital having turnover of 45-50 patient per day and equipped with bedside noninvasive monitoring system, crash cart, resuscitation suite and minor operating theatre. I am working as an Attending Consultant for the department and doing all the resuscitation / triaging independently. We often see mix cases like Pediatrics, Orthopedic, Surgical, Cardiac, Medical, Trauma, Neuro Surgery, Neurology, Obs. Gynecology, ENT, Eye Emergencies etc.

The procedures which I often do independently are Rapid sequence Intubation, Surgical Airway management, Suturing, Incision and Drainage, CVP and Arterial line insertion, ABG Sampling, IV Canulation, Surgical and Burn wound management etc. We did several Thrombolysis for acute MI under supervision of Cardiologist. Recently we started doing ultrasound for FAST study in trauma and Non trauma patients.

The 3yrs Masters in emergency medicine curriculum was taught along with the shifts. The curriculum include daily class to cover the particular module power point presentation by fellows and faculty, bed side teaching, clinical evaluation, research work,log book, pre and post test every month, hands on training, simulation classes etc. I have finished 3 yrs of the curriculum successfully.

Clinical Experience in Surgery in SSRN Hospital, Mauritius: SSRN Hospital, Mauritius is 700 beds Multi-Specialties hospital under Ministry of Health. While working as a Medical officer, I was posted in General surgery, Neurosurgery, Accident & Emergency and Cardio-Vascular surgery. During these posting, I got the opportunities to assist in various minor and major surgeries like Incision & drainage of Abscess, Mastectomy, Laprotomy, Nephractomy, Trans urethral resection of Prostate, Craniotomy and evacuation of Subdural Hematoma, Burr Hole and removal of Extradural Hematoma, Ventriculo-Peritoneal Shunt etc. I performed independent procedures like Intravenous cannula insertion, Pleural Tapping, Intercostal chest tube insertion, central venous pressure line insertion etc. I was attending outpatient department routinely with my specialist .I have managed the pre and post- operative patients of CABG and Vascular surgery under supervision of Cardiac surgeon and Cardiac Anesthetist. I got the opportunities to resuscitate the patients in Cardiogenic and Anaphylactic shock. I have attended the biweekly departmental CME .

Clinical Experience In Neurosurgery Intensive Care Unit : I worked as a resident doctor in eleven beds Neurosurgery ICU of Indraprastha Apollo Hospital which is a corporate Multi specialties hospital in north India. Out of eleven beds, 6 beds are for polytrauma patients including head injury with a huge turnover. During this posting, I assisted the anesthetist in the active management of all critically ill patient and Initiation of elective ventilation with uses of muscle relaxant & sedation and performed Minor procedure like Cannula insertion, Catheterization, inter costal chest tube insertion, pleural aspiration

,lumbar puncture, Intracranial Pressure Monitoring, Venesection etc. The Maintenance of hemodynamic, Invasive monitoring through arterial line and CVP line were the routine jobs. I also learned the weaning of critically ill patients from the mechanical ventilator and interpretation of arterial blood gas report.

Clinical Experience in pulmonary and critical care unit: After completion of internship, I have worked in tuberculosis and chest department of LRS TB Hospital and Safdarjang Hospital, India. During that period I have acquired various skills for management of critically ill patients on mechanical ventilator. I did minor surgical procedures and learnt nebulisation and peak flow meter uses. During posting in tuberculosis department, I managed several patients under my seniors supervision, who was on first line and second line anti tubercular treatment.
