Ensures that reasonable expectations of client care are met through the coordination of physician, patient, family, external case manager, insurance adjuster and staff. Team building, implements plan of care for patients in home setting; monitors ongoing care, compliance with care plan, and progress towards established goals for effectiveness and appropriateness of care and satisfaction with services.
