Provided 5-point decisionmaking process in the appropriateness of admissions into a community level Mental Health Kokua adult mental health group home: 1) Maintains requirements of the State of Hawaii Department of Health Adult Mental Health Utilization Management office and federal Health Insurance Portability and Accountability Act through a Sharepoint platform; 2) Submits a clinical review and BioPsychSocial meta-anlayses of Hawaii State Hospital Patients who are mentally unfit of criminal sentencing for jailable offenses; 3) Evaluates comprehensive background, crimogenic involvement, and risk for relapse; 4) Completes, submits and discusses findings in a group setting with housing staff and community case management agency; 5) Makes recommendations for additional support services to assure successful placement in the lowest level of community housing available.
Gave direct clinical and administrative supervision to 15 on-site staff, and 41 dually-diagnosed single adult individuals with severely persistent mental illness (SPMI): Therapeutic Living Program has 14 beds; 24hr program has 7 beds; 8-16hr program has 12 beds; and Semi-Independent program has 7 beds. Housing program deliverables focus on a residential setting with clear expectations given by a judge to an SPMI patient to assist with the maintenance of community custody as managed through a probation officer. This judge assigns SPMI patients on Conditional Release or Release on Conditions to a case management agency for placement at an MHK housing program.
Worked collaboratively with case management agencies and forensic coordinators to enforce the legal conditions of the consumers’ community custody level by linking these SPMI consumers with community services to increase their protective factors and reduce their risk for re-offending, and/or re-hospitalization.
Interpreted the Individualized Level of Skill Survey (ILSS) and Level of Care Utilization System (LOCUS) to assign the most appropriate, lowest level of community housing support and supervision for SPMI patients in tenancy with MHK.
Trained residential assistant staff in Trauma Informed Care, Motivational Interviewing, Using “I” Statements, Cultural Competency, Customer Service, and documentation skills. Used the NextStep program to develop consumer crisis plans & treatment plans, which trends, tracks and monitors consumer progress or lack of progress in specific activities of daily living.
Assessed consumer motivation to change behavior, and willingness to participate in MHK housing programs through individual counseling and implementation of the Behavior Contract, Housing Action Plan and Termination of Program Services as reflected by the dynamism of the consumer.
