Assess the client’s health condition and the home in which the client resides. Read the doctor’s orders and check the home for accessibility issues.

Create a care plan for that client. The care plan is followed by aides and other home health employees who visit the client. Set up and maintain the clinical record for that client.

Create a schedule to visit clients on a regular basis to review the client’s progress and to provide those services only a registered nurse can provide, such as checking glucose, medications reconciliation and checking vital signs. Documentation of falls, emergency calls, observations and out-of-the ordinary behaviors is maintained by the nurse in the client’s chart and shared with the care team.
