Outpatient medication management of patients with diverse needs

-Prescribed psychiatric medications for patients age 18 y/o up to geriatric patients in a small specialty outpatient clinic serving the New West network of primary care clinics. Patients referred by their primary care provider for medication management. Met with patients in a hybrid model of in person visits 2 days a week and by video visits from home 3 days a week.

-Performed 60 minute new patient intakes and 30 minute followup visits to diagnose and treat symptoms all AxisI disorders, Personality Disorders, Substance use disorders and eating disorders.

-Documented intake and follow up visits to reflect history of medication use, treatment modalities and ongoing progress in follow up visits. Differentiated between psychiatric symptoms caused by or exacerbated by medical problems or substance abuse.

-Developed a treatment plan with patients, including discussion of diagnosis, rationale for treatment with medication and therapy. Obtained informed consent for medications and provided education with use of medication and how and when to report change in symptoms or side effects..

-Referred to therapists in the network and in the community. Referred patients at high risk for self harm to the ER for evaluation for inpatient psychiatric admission.

-Treated patients independently but consulted with supervising psychiatrist as needed for best patient care. Collaborated with the clinic's 2 psychiatrists and 2 therapists during weekly video treatment team meeting. Covered prescription requests and any emergent issues as needed when psychiatrists on vacation.
