EMPLOYMENT:

During my time with Kindred Hospice I have worked completing F2F’s, certifying and re-certifying patients for hospice care.

Crossroads Hospice & Palliative Care

14310 E 42nd St. South, Unit 600

Independence, MO 64055

5/15 to 8/21

During my time with Crossroads I have worked simultaneously as a palliative care and hospice nurse practitioner in the community. I have also worked at Centerpoint Medical Hospital as a palliative care nurse practitioner. I have worked closely with a social worker and chaplain and have had the opportunity of caring for patients in their own environment and in the hospital, I have learned how different cultures address the end of life experience, worked autonomously addressing all medical aspects of their health treatment, and learned pain therapy with palliative and hospice patients. I have had the opportunity to know the patients and their families which has been a wonderful experience. Working in the community has increased my awareness of community needs of patients and increased my interpersonal skills. Working in the hospital has only re-enforced my knowledge of understanding how patients can reach their best possible outcomes.

Truman Medical Center

2301 Holmes Street

Kansas City, MO 64108

12/09 to 5/15

As a nurse practitioner in palliative care I was responsible for the day to day operations of the team. The team is composed of social workers, spiritual health, registered nurses, and physicians. Patients are seen in an acute in-patient setting. We rounded on patients as a team as well as individually. I completed a detailed physical assessment and reviewed the ECG, labs, vital signs and the medical history to select the “best” plan of care with and for the patient. Thus, keeping the patient, family, and team involved for the best possible outcome.

During my tenure, I led the team to Joint Commission Certification. We “won” a $10,000.00 grant for the application fees, were the 33rd in the country to be recognized as a joint commissioned site, remain the only site in Missouri and one of the few in the Midwest. Recognition by joint commission did not surpass the recognition from my peers who recognized me by awarding me with the Advanced Practice Nurse of the Year award from Truman Medical Center.

In my role, I was responsible to hire clinicians to meet the patient’s goals and expectations around the quality at the end of their life. I established policies on the use of evidence based clinical practice guidelines, updated all clinical policies and practices, and based my practice on these guidelines.

Kramer & Crouse Cardiology

Cardiovascular Consultants

2500 Glenn Hendren Drive

Liberty, MO. 64068

5/02 - 12/09

In this role, I worked as a cardiology nurse practitioner in a busy office. I had my own panel of patients within a 4-physician run clinic. I completed physical exams, ordered acute and yearly cardiac testing, labs and ECGs. I oversaw stress testing and I completed pacemaker and defibrillator checks.

I also worked as a research coordinator for the cardiologists who were sub-investigators in multiple cardiology trials. I assisted in developing standards and processes to provide consistency in evaluating and selection of subjects for participation in clinical trials using current evidence. I completed a detailed physical assessment and reviewed the ECG, labs, vital signs, and medical history to select the best subjects for participation in clinical trials specific to the investigational product and the protocol. Monitored “on study” subjects for adverse events and documented and treated any adverse events using evidenced based practice and current ICH documentation regulations.
