Monitor timesheets, travel vouchers and attendance.
Maintain necessary liaison with providers of health services to assure the provisions of service to carry out medical recommendations; securing necessary diagnostic, medical and treatment services; arrange for obtaining physician or hospital services in an emergency.
Ensure the evaluation of participant’s progress, utilizing the staff that provides direct services.
Ensure that the centers provide the appropriate number of days of service determined by the funding source and the Medical Assistance budget.
Act as liaison for the Division Advisory Council, assist in agenda preparations, membership maintenance and materials distribution.
Supervise the record keeping for the center that is required by agency policy, such as personnel evaluations, records of client attendance and progress, health and safety inspections, time records, and all training and related data.
Monitor participant files to ensure continuum of care and compliance with funding source and licensing requirements.
Establish and maintain cooperative relations with appropriate community services and facilities to meet the needs of the elderly and handicapped.
Pinpoint problems in the area community and plan a means of helping the target population overcome obstacles.
Maintain liaison with community and neighborhood institutions, organizations and ethnic groups.
Develop and establish a Quality Assurance system for evaluating program effectiveness and the effectiveness of the individual health care plans and interventions
Develop a monitoring plan and assign components to center staff.
Perform random audits of client records for compliance with regulatory guidelines.
