I am a current CPhT and have 35 years of pharmacy experience. For the past 8 years l have worked in our busy Emergency Department as a Medication Reconciliation technician. I reconcile patient medication profiles. My duties include getting a complete and current medication history of the patients. I update their profiles with patient and family. • I entered Rx prescriptions and set delivery dates and times. The Prialt Navigator Program became available all across the country. When I started working there in February of 2013 we were a new program with maybe 25 MDOs and a few hospital pharmacies, before I left we were working with 425 medical doctor practices, many hospital pharmacies and universities, just to name a few: OSU, Kaiser, Suny, Boone Hospital, Stanford, Cancer Centers of America, Henry Ford Medical Center, Moore Cancer Center, Walter Reed Medical Center, Cleveland Clinic, etc.
• I also helped patients with co-pay assistance programs that met criteria for assistance.
• I am very knowledgeable with pharmacy software programs as well as MD Clinics software for physicians and medical billing.
• I also previously helped establish a medication dispensary at the offices of Medical and Surgical Associates. I incorperated new patients, contracted with insurance companies, filled prescriptions, screened for drug allergies, drug interactions.
• I have helped patients with compliance and understanding of their medications, all under supervision of physicians.
