This letter contains important information about accessing your USIC pay statements, updating your personal contact information, and information regarding your benefits at the conclusion of your employment with USIC. Viewing USIC Pay Statements & Updating Your Address for W2
(*Note – You must use either Google Chrome or Firefox web browsers to access Workday*) To view or print your USIC pay statements (July 2013 – present), or to update your contact information to ensure delivery of year-end tax documents is not delayed, please log into the Workday system with the following information (you will no longer have access via Single Sign-on): Workday URL: https://wd5.myworkday.com/usicinc/fx/home.flex?redirect=n
• Username: Your first and last name, all lower case with no space between (example: larrylocateman)
• Password: Workday$ + last 4 digits of your SSN (example: Workday$7890) If this does not work, contact the Payroll Helpdesk at 877-***-**** to reset your password.
• Pay Statements/W2 – Once logged into Workday, these documents can be found in the Pay application under Payslips and My Tax Documents.
• Updating your Address - Once logged into Workday, click on Personal Information
Click on Contact Information
Click Edit and proceed to make the appropriate changes
Be sure to click Submit for the changes to go into effect PTO Payout
Any accrued and unused PTO will be paid on your last paycheck. Health Insurance Coverage
Health Insurance coverage ends at midnight on your last day of employment with USIC. It can take 2-3 weeks to receive COBRA paperwork after your last day. This paperwork will explain how to continue your health insurance and the cost of continuing coverage. If you have not received your paperwork after 2-3 weeks, please contact our COBRA provider, Wex Inc., at 877-***-****. • Health Savings Account (HSA)
These are individually owned accounts; therefore, your account will stay open after your last day worked. You will also still have access to your bank account.
You may choose to (1) transfer the funds to a new bank, (2) leave the account open with Mercer Marketplace (maintenance fees may be assessed), or (3) request a distribution using the HSA Distribution Request Account Closure Form (taxes may be incurred).
• Dependent Care Flex Spending
Dependent Care FSA debit cards will be shut off after your employment is terminated. You have 90 days following your last day worked to submit any claims that were incurred prior to your termination date.
401k Account
Distribution options include cashing out your account, rolling it over to another qualified plan, or if your balance is greater than $1000, you can keep the account with Principal Financial Group after you leave USIC. If your balance is less than $1000, you will be required to take a distribution. Contact Principal Financial Group at 800-***-**** to discuss or visit their site online at www.principalfinancial.com. Registration may be required (USIC SSO login no longer available).
401k Loan
To obtain more information on your loan balance, you may contact Principal Financial Group at 800-***-****.
• If you have an active loan, the balance must be paid within 90 days of your last day worked or your loan will be placed in default. Default means a 1099 tax form will generate in January of the following year for tax purposes.
• If you take a 401k distribution, AND have an outstanding loan balance, your loan will automatically be defaulted. The unpaid loan balance is considered an early withdrawal and a 1099 tax form will be generated for the remaining unpaid balance.
• Upon termination, a lump sum payment can be made in the form of a cashier’s check or money order; please contact Principal Financial for detail information on this process. Life Insurance
If you met the 90-day waiting period during your employment, you can convert/port your USIC group term life insurance policy to an individual policy. If you are interested in conversion/portability cost and want to discuss your options, contact Lincoln Financial Services at 877-***-****.
