Responsible for reviewing prior authorization requests for clinical appropriateness of the following: enteral feeding; attention deficit disorders; growth hormone; red blood cell count deficiencies; and TPN evaluations, . Evaluation of over utilization of emergency department visits and hospitalizations in the disease specific areas of asthma, cardiovascular disease, diabetes and mental health and how to develop a management system to reverse this trend. Development of a smoking cessation program for woman who takes birth control pills, smoke and utilizes Title V family planning clinics. Review Family planning grants for Title X clinics. Make grant recommendations for the Title X clinics.
Consultant to the director of the asthma care initiative, for development and implementation of the strategies for a state initiative in the area of asthma care management.07/01/98-2010. Assisted in writing the change of ODH/BCMH rule that identifies pharmacists as providers.
Developed a Preferred Drug List (PDL) for the BCMH drug formulary. Responsible for the semi- annual update review and amendments to the Nutritional Supplement Formulary for BCMH.
Wrote guidelines for the implementation of Medication Therapy Management Service Codes (MTMS) for Ohio Department of Health the Bureau for Children with Medical Handicaps. Established compensation rates for these codes most recently promulgated by the AMA for use by pharmacists. Codes assigned compensation rates effective 07-01-05. Codes will be used by pharmacist providers to bill for MTM services provided to pediatric asthma patients and diabetes patients covered under BCMH. The pharmacist provider must be working in collaboration with the child’s managing pediatric pulmonologist, pediatric endocrinologist or pediatrician. First Government entity to assign compensation rates for MTMS.
Pharmacy liaison to the Executive Director, Ohio Commission to reform Medicaid, from ODH.

Amylin Pharmaceuticals: Contract consultant through a Master Services Agreement. I am responsible for review and suggestions for modification in the PARC slide deck for Symlin, promotional presentations. These modifications have been submitted to PARC and are under review for final disposition. I have also been engaged to review current effectiveness and to look at the distribution issues surrounding the product Symlin. 2011 : Member of the Symlin team’s speaker bureau to present promotional talks on specific clinical information as it relates to the product Symlin. Talks have been to date in both Ohio and Florida. 2011-2012

Director: Diabetes Management Program: Responsible for the overall patient care management, coordination of pharmaceutical care services and disease management systems for the medical practice. Areas of specific patient care are diabetes, lipid metabolism disorder d essential hypertension, metabolic syndrome, CAD, CHD and PVD for Grandview Family Practice. Direct the multi disciplinary team of physicians, nursing staff, dietician and athletic trainers in the management of patients with diabetes and or any of the aforementioned co morbidities. Responsible for suggestions in the area of medication drug therapy management. Evidence based presentations on drug therapy dependent upon laboratory results and vital signs. Work with patients and team members in the modification of patient behaviors surrounding diet, exercise and foot care. Achieve clinical goals in two-thirds of patients, at approximately six months. CHIEF EXECUTIVE OFFICER
Developed and initiated pharmacist service network (PSN) dedicated to performance outcomes. The function of STRATEGIC HEALTH OUTCOMES, INC. (SHO) was to provide clinical pharmacy services to MCO’S, private insurers, et al and capture and report pharmacoeconomic data.
Assisted in the creation and lobbying of landmark legislation (P.L. 1995, chapter 331) mandate of payment to qualified pharmacists who provide diabetic education to identified patients from private insurance (1/96).
Assisted in the drafting of H.R. 3757 (The Safe Medications for the Elderly Act) federal bill to amend the Social Security Act, to identify pharmacists as health care providers and allow them to be compensated for services rendered (proposed-U.S. Congressman Frank Pallone) filed 7/96.
Received a favorable opinion from the Federal Trade Commission, Bureau of Competition, allowing pharmacists to create service networks, negotiating at risk contracts for said services with insurance companies and like entities. Opinion granted, summer of 1997.
Collaborated with “Little Phrma “in New Jersey on a regular basis both on professional and government affairs issues.

MANAGING DIRECTOR
Created the Center for Pharmaceutical Care and Research (CPCR), an educational nonprofit foundation of the New Jersey Pharmacists Association consisting of pharmacists who practice pharmaceutical care. CPCR is an educational consortium, which has trained over 250 pharmacists in the areas of diabetic and asthma health education training. The Consortium consisted of five colleges of pharmacy, PCP, Temple, St. John‘s, Arnold and Marie College of Pharmacy (LIU), and SUNY at Buffalo. Assisted in teaching, curricular development and administration of the courses described above.
Wrote a grant proposal along with Dr.Gary Erwin PCP&S, to the Grotta Foundation. The purpose was to study the affects of pharmaceutical care in elderly diabetic patients in Essex and Morris counties, state of New Jersey. The grant was received 6/1/95 with a submission date of 9/1/96 and a Poster session at the ASHP midyear clinical meeting on 12/96. The document was used as the primary study tool for the federal legislation mandating care and payment for Medicare patients suffering from diabetes.
