2. Proof of ownership of the property (Title if available) 3. Copy of current driver’s license (date of incident) 4. Full view photos of the damaged property

5. Photos of the area where the incident/accident occurred 6. TWO (2) repair estimates or a paid receipt for repairs 7. Proof of current insurance (date of incident)

8. Proof of current registration (date of incident) 9. Police Report (if applicable)

Failure to provide all of the requested documentation or an incomplete damage report could delay the processing of your claim. Additionally, submitting this information does not guarantee that payment will be made on your claim.
