AUGUST 2016-PRESENT

Independent clinician specializing in geriatric counseling services. Counseling services are provided to the patient at their residence; nursing home, assisted living, private home. Counseling services address issues related to health status, chronic pain management, adjustment to new living situation, grief, and loss, as well as symptoms related to depression and anxiety.

From March 2016 until August 2016, I was unemployed and searching for a new opportunity that would allow me to utilize my clinical skills and offer me flexibility in my schedule in order to prioritize personal and family issues that my previous employer was reluctant to accommodate.

COUNSELING SERVICES SUPERVISOR/ HOSPICE OF SOUTHERN ILLINOIS

FEBRUARY 2009- MARCH 2016

Supervision of 6 medical social workers and 2 bereavement counselors. Worked closely with the team to increase compliance with Medicare mandated assessment timeframes to 100%. Participated weekly in Interdisciplinary Team Meetings; which consisted of social workers, bereavement counselors, nurses, volunteer coordinator, as well as the medical director. Ensured that the needs of patients and families were being met; advocated for patients/families; and educated other team members regarding psychosocial issues. Participated in Continuous Quality Improvement activities; provided monthly compliance and productivity reports to Clinical Services Manager. Covered cases on an as needed basis for social workers and bereavement counselors.

PROGRAM MANAGER/ ILLINOIS MENTOR

January 2001- November 2008

Oversight of four programs in two different geographical locations (O’Fallon, IL and Marion, IL). Developed three of the programs as new start programs, after assuming the role of program manager. The original program was exponentially increased from 12 children served, to over 150 children served. Responsible for budget development and oversight for all four programs. Other responsibilities included all aspects of hiring, disciplinary action, evaluation of employee performance, and trained staff and foster parents. As part of the Continuous Quality Improvement Program; collected, compiled, and analyzed data, assessed, and applied to make programmatic and/or budgetary changes.

PROGRAM COORDINATOR/ ILLINOIS MENTOR

April 1999-January 2001

Responsible for case management of special needs foster children; which included writing detailed treatment plans tailored to each child’s needs, and tailored towards their court determined permanency goal; as well as writing detailed court reports. Required court appearances, as well as collaborated with community agencies to secure necessary services for clients, and to monitor progress.

MEDICAL SOCIAL WORKER/ HOSPICE OF SOUTHERN ILLINOIS

August 1997- April 1999

Performed comprehensive psychosocial assessments of patients/families to ensure that their needs were being met. Met with family members after patient’s death to assess their grief response, coping skills, and to provide therapeutic intervention. Weekly participation in Interdisciplinary team meetings in order to ensure that patient’s/family’s needs are being met, and to advocate for patients/family. Served as the Clinical Consultant to the Director of Counseling Services.

Provided intensive, in-home services to families with children at risk of being placed in foster care due to abuse or neglect.
