DIRECTOR OF NURSING

As the DON I am responsible for managing the well being of all residents by ensuring compliance with state and federal laws, and facility policies and procedures. I collaborate with other departments, medical professionals, consultants, and organizations, including government agencies to develop, support, and coordinate resident care, and related administrative functions in representation of the community.

Key Responsibilities:

Participate in the recruitment, interviewing, and selection of nursing and care staff personnel.

Council team members when required in a fair and consistent manner while adhering to The Plaza policies and procedures and state / federal employment laws.

Supervise nursing / care staff and regularly evaluate their performance.

Conduct regular shift reports, daily meetings, and other modes of communication to disseminate information, interpret policies and procedures and address concerns as needed.

Ensure resident nursing department and work areas are clean, neat, and orderly. Develop and provide ongoing in-service training for all care staff as relevant to the needs of the residents and the community.

Perform patient/resident assessments in coordination with administration and marketing staff for each resident, prior to admission to determine if the prospective patient/resident care needs can be appropriately met by our facility.

Create patient/resident service plans: using information gathered during the assessment process I develop care communication for patients/residents, family members, and staff.

Conduct routine assessments related to resident service plans to include initial assessment prior to admission, 30 days post admission, bi-annually, and during acute changes in condition.

Facilitate and encourage resident and family participation service or care planning by explaining alternative choices and involving them in educational programs.

Oversee nursing and care staff practices, to ensure care will be provided with the best interest of the resident in mind, and to ensure prevention of complications, and promote independence with a high quality of life.

Manage care implementation by nursing or care staff; ensure proper documentation of care delivery as ordered by the physician in accordance with federal requirements, state regulations, and Plaza policies and procedures; recognizing any deviation from resident service plans, department of health standards, and Plaza policies and procedures.

Monitor resident nutritional status to ensure care needs are met in accordance with resident service plans, department of health standards, and Plaza policies and procedures.

Perform rounds frequently throughout the day to observe efficiency of service delivery; interview staff, residents, family, or other parties of interest as appropriate.

Maintain medication management programming; assessing individual residents to determine ability for self-administration of medication, providing oversight of medication storage and administration as appropriate.

Routinely monitor and audit medication management program and ensure medication/treatments are administered and documented by nursing and care staff as ordered by the physician in accordance with federal requirements.

Ensure medications are ordered and received from the pharmacy in a timely manner.

Develop and oversee health promotion and disease prevention programs that include administration of immunizations as appropriate and development of protocols for infectious disease management.

Participate in daily and weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints and concerns to maintain a high quality of care and life for residents.

Notify family and physicians of significant changes in resident care/ well being and take appropriate action.

Ensure proper contact with physicians occurs regarding written and verbal orders to include accurate transcription of physician orders and timely follow up as needed.

Assist with making necessary and expedient arrangements for consultants, laboratory, and x-ray as necessary.

Perform direct resident care as necessary and appropriate.

Promote customer service and hospitality and respond to and adequately and resolve complaints or concerns from residents or families about nursing services.

Monitor facility incidents and complaints daily to identify those defined as unusual occurrences by state policy and promptly report such occurrences to the administrator for appropriate action.

Promote compliance with accident prevention procedures, safety rules, and safe work practices to prevent team member injury and illness.

Ensure adequate supplies are maintained for nursing and care staff office, submit request for supplies as appropriate for departmental budget by community.

Collaborate with internal, third party, and state inspectors visiting the community.

INJURY PREVENTION GROUP, SAGINAW, TEXAS 2019 - 2020

OCCUPATIONAL HEALTH AND SAFETY NURSE (OHN)

I worked closely with plant safety and human resources departments to reduce workers’ compensation claims, minimize the company’s exposure to work related injuries and illnesses, eliminate lost time, improve productivity, and thereby save time and money. I am trained in differential triage which enables me to determine the best course of treatment. I performed certain treatments on-site which allowed me, and the doctor I work directly under, to control, direct, and manage the outcomes.

Key Achievements:

Successfully implemented and managed the above-mentioned program with CTI Foods in Saginaw, TX.

With my services the company managed to reduce its workers’ compensation claims by 40% in the first 6 months.

While with IPG I obtained a Basic Life Support Certificate, and I am also certified to teach this course.

FAITH COMMUNITY HOSPITAL, JACKSBORO, TEXAS 2013 - 2015

HISPANIC OUTREACH COORDINATOR / COMMUNITY LIAISON / TRANSLATOR

They were always more relaxed and understood the information, and could better communicate their symptoms, when I was with them in person.
