SIGNIFICANT ACHIEVEMENTS

Reported the First Ever Serologically Confirmed Rubella (German Measles) outbreak in India. In January 2006 to 22-04-2006, Total 277 cases of suspected measles cases reported from Mangasuli village.

ERT (Epidemic Response Team) comprising of CEO Belgaum district, SMO NPSP WHO GOI Unit Belgaum, DHO Health and Family Welfare,RCH Officer Health and Family Welfare Department and Principal Jawaharlal Nehru Medical College Survey was done on 1st February 2006 to 2nd February 2006.

Total 999 houses and 5,136 population survey were done.

Five blood samples and Two urine samples collected and sent to King’s Institute of Preventive Medicine Chennai in Reverse cold Chain.

All Five Blood samples Came Positive for Rubella (German Measles),

Two morning urine samples are at CDC Atlanta, for genetic sequencing, results awaited.

INFERENCES:

A Disease Surveillance System involving auxiliary nurse midwives, Anganwadi workers, Community leaders, Local practitioners, Private Nursing Homes, Trained birth attendendants, teachers both Government and private schools Referral hospital staff and consultants private and Government was established.

Reporting of suspected cases of anyone suffering from Fever with Cough, Coryza, Conjunctivitis and Rash was strengthened by Community Awareness Campaign.

Since it is the first ever serologically confirmed report of rubella in India.

The proportion of infant deaths due to congenital heart diseases in Belgaum District is 11.7% whether a significant proportion of these deaths are due to rubella needs to be explored.

A larger community-based surveys may be undertaken, to Confirm presence of Rubella in communities.

The findings of such survey might have policy implications regarding the inclusion of measles mumps. Rubella vaccine as part of universal immunization Programme.

The surveillance results were published in National News Papers which lead to the inclusion of Rubella vaccine Inclusion in the Routine Immunization Programme.

1.Innovated and implemented REACH THE UNREACHED Polio Campaign on 6th January and 10th February 2007.

2.The Campaign was Aimed at covering 0-5 children from Migratory Population especially from Maharashtra Bheed Taluka (Sugar cane cutters families) where Wild Polio Virus 3 was active.

3.The challenge was to Identify, Locate, Reach and Immunize these 0-5 years’ children. Because most of the sugar cane cutters and other migratory population were located in very remote fields and hamlets and they work from 7.00am to 6.00pm in their fields.

4. They are only available from 6.00 to 10.00pm. (NID ON SUNDAY IS FROM 8 AM TO 5PM)

5.The aim was to Stop the Wild Polio Virus Transmission in our community.

STRATEGY:

Ten Additional Teams comprising of One vaccinator and Two community leaders, like Gram Panchayati Vice-President, Members, Local Leaders and Veterinary Staff reached their respective areas By bikes and some remote areas by walk starting at 6.00pm to 10.00pm on 6th January and 10th February 2007, SATURDAY (PRE-BOOTH ACTIVITY).

RESULTS:

6th January 2007: 995 migratory families covered and immunized 670 children.

10th February 2007: 452 families and immunized 409 children.

6th January 2008 560 migratory families from Maharashtra and immunized 437 children

6.Rendered medical services in the relief camps to the people of Gujarat in Bhuj District who were affected by earthquake during February 2001.

a)The job Responsibility involved working in Most remote and Hard to reach areas as Khagaria was considered as epicenter for Wild Polio Virus transmission in India mainly surrounded by seven major rivers Ganges, Baramathi, Kosi, Kali kosi and Ghandak.

b)One third of the district population residing in hard-to-reach remote areas.

c)Khagaria district has highest population density, very low literacy virtually no sanitation and high degree of protein energy mal nutrition and high incidence of diarrheal disease in the 0-to-5-year age group.

d)Monitoring and surveillance of SIA activities of April, May and June 2008 Pulse Polio rounds especially in Beldur block which was having wild polio virus 3 transmission.

e)Did the Field validation of riverine and hard to reach areas of Khagaria district especially along Kosi river.

f)Trained 491 vaccinators and 73 supervisors for supplementary immunization activities during above 3 rounds.

g) Planned and conducted regular meetings with Government Public Health counter parts and UNICEF for smooth implementation of Polio National Immunization Days and Routine Immunization in the district.

h)Investigated 15 AFP cases and did 60 day follow up of 8 cases during the tenure.

i)Involved the Borderless and Grid Monitoring between Khagaria, Madhepura and Saharsa district.

3)Worked as Community Co-coordinator in “Evaluation of an Emergency Obstetrics and Newborn Care Intervention Package to Reduce Adverse Pregnancy Outcomes in Low Resource Settings (The EmONC Trial)" tested an integrated package of strategies for improving pregnancy outcomes.

The strategies included creating awareness in the communities for birth planning and hospital transport, birth attendant training for early recognition of pregnancy and newborn complications, and hospital staff training in the management of obstetric and neonatal emergencies. Lessons learnt from this project suggest that achieving improvement in pregnancy outcomes in these settings will require substantially more obstetric and newborn care infrastructure and provider training and community mobilization alone will not be sufficient.

Trained 74 community facilitators in Community Mobilization and Home-Based Life Saving Skills.

Trained all Staff nurses working in Community Health Centers1) Gokak, 2) Bailhongal,3) Athani, 4) Chikkodi and Bailhongal in Essential Newborn Care and Neonatal Resuscitation and AMTSL, Partogram, Management of Obstructed labor and APH and PPH.

4)Worked as Field Research Officer in the ‘Maternal Newborn Health Registry (MNHR)’ an essential component of the Global Network, is a prospective, population-based observational study to quantify trends in pregnancy outcomes, including stillbirths and neonatal and maternal mortality rates, in geographically defined low resource areas. It has operated since 2008, enrolling all pregnant women residing in defined study clusters and tracking birth outcomes through 6 weeks post-delivery. As a part of registry, annual household survey is undertaken and all the eligible couples and Likely-to-Conceive women enlisted at every village level. They are being tracked by Accredited Social Health Activist (ASHA) and Registry Administrators (RAs) every month for early identification of the pregnancy and subsequent enrolment. This unique systematic tracking has been the baseline for many research protocols targeted to test the interventions for reducing maternal and newborn morbidity and mortality. J. N. 5) Worked as Community Co-coordinator in the “First Breath: Community Based Training and Intervention in Neonatal Resuscitation” project was implemented at seven Global Network Sites located in South Asia, Africa and Central and South America including J N Medical College, Belagavi.

In order to address the issue of birth asphyxia, one of the leading causes of early newborn mortality, the 'Global Network' between 2005 and 2008 launched a major initiative of training community - based birth attendants in Neonatal Resuscitation for reducing early neonatal mortality in developing countries.

Nearly 90 communities, 26 of them in Belagavi District, defined as geographical medical service-based area with approximately 500 births per year, participated in the study aimed at comparing the efficacy of Neonatal Resuscitation Program (NRP) of the American Academy of Paediatrics with the Essential Newborn Care Course of the World Health Organization (WHO) in reducing early neonatal mortality.

More than 65,000 births, approximately 26,000 of them in 298 villages with a population of more than 890,000 in Belagavi District, were assessed over the two- and half-year study period at the participating GN sites.

As part of the trial, over 1,100 birth attendants including Private Practitioners, Nurses, Auxiliary Nurse Midwives and Traditional Birth Attendants were trained for newborn resuscitation on three occasions.

JOB RESPONSIBILITIES:

6.Developed check and counter check form to ensure not a single event is missed in the community.

7.Trained 46 birth attendants in the community for baseline data collection study and then in the WHO ENC guidelines for Newborn Care: Training included obstetricians, traditiondais, local doctors, general practitioners and ANMs.

8.To examine every newborn within 24 hours after delivery and on the 7th day and CNS examination of the newborns who were resuscitated with bag and mask.

9.Developed 1st line of information and second line of information to ensure timely information of events and not miss a single event.

10. Developed checklist for ENC monitoring and implementation in the community.
