Key Duties and Responsibilities:

Taking part in continuous quality improvement projects for continuity of care, patient retention, and adherence support, currently on a retention project.

A case manager to clients with varied health needs. i.e. new clients viremic cases and clients with various support needs.

Formulating Strategies for proper management of client’s and mentor facility peer educators on strategies for continuity of care.

Taking part in MDT to discuss clients with high viremia by summarizing the files, before presentation.

Ensure that a list of daily appointments was generated to identify the missed opportunities and clients followed up on a daily basis to ensure no missed appointments towards 95% retention.

Ensure complete documentation was done on the hybrid system for continuity of care tools (registers, FMATT and FIIT etc).

A team lead in the implementation of Differentiated standard care Model and monitoring clients both on facility and community.

Ensuring daily/weekly/monthly/ quarterly updates to the retention technical person on daily/weekly activities and identify challenges so that they are addressed on time, or weekly meetings.

Assisted in solving IT problems related to EMR whenever issues arouse in Mbagathi Hospital.

Addressed complex adherence and GBV cases at the facility addressed successfully.

Supported FACES program in designing and development of Ushauri system; an electronic data management system for monitoring patient’s appointment to facilitate 95% retention across FACES supported sites in Kisumu County as a TOT the system is fully adopted by NASCOP for use across HIV programs in Kenya.

Maintained the facility at 85% to 90% retention despite the facility being a high-volume site as a retention champion in FACES-Kisumu County Hospital.

Developed an action plan that helped in monitoring the program deliverables; which saw the program achieved 98% of its objective in 2017.

As a rider, I ensured all the treatment interrupters for 2017-2018 cohort for Lumumba and Kisumu County Hospital site were successfully traced at 95% with documentation done.

Prepared weekly budget for timely running of the departmental activities.

Represented the department at higher forums where more technical issues affecting the team were presented and addressed.

2017-2018 Family Aids Care Education and Services-Kisumu County Hospital

Position: Facilitator in HIV/ AIDS and Adherence

Conducted sensitization and training to Kenya Plant Health Inspectorate Service (KEPHIS-Kisumu) staff on HIV/ AIDS in the following areas: Family testing, disclosure, adherence to inform knowledge of status among the staff members as guided by their organizational policy.

Selected Accomplishments:

90% of the staff turned up for status knowledge.

The organization made it a culture, that annually staff members were sensitized on HIV/AIDS contemporary issues as a result of the training impact to the organisation.

2013 - 2017 Sunburst project-FACES Kisumu County Hospital

Reinforcing adherence to medication while addressing their psychosocial needs and supporting them on disclosure issues.

Fighting discrimination in school that has escalated stigma of adolescents by creating a student-teacher friendly environment through health talks in school.

Sensitization on reproductive Health issues affecting the youths e.g. HIV/AIDS in schools, churches and adolescent camps.

Empowerment on Human rights by bridging the gap between the youths and adolescents, for equity in representation at the local and national level.

Managed to reach most of the schools within Kisumu town both primary and secondary sensitizing them on behaviour change.
