Acts as role model and conduit for information between staff and nursing administration and represents administration objectives of nursing services to residents, staff and families. Establishes and individualizes unit routines unit to ensure best practice and unit cohesion. Completes disciplinary actions with support from nursing administration. Investigates and follows through on issues and deficiencies noted, makes plan of improvement and documents issues per protocol. Communicates relevant observations including changes of condition, abnormal laboratory and x-ray results to appropriate colleagues and documents appropriately. Participates in MDS process, completes MDS observations and participates in interdisciplinary care plan meetings. Resource for resident information for families. Communicates clinical information accurately and compassionately to families, assists them in decision making and meets their individualized needs and concerns in the process.
