Community Health Worker – Supervisor - Pediatrics

Community Liaison Workers, under supervision, serve a members of interdisciplinary teams and provide a bridge between the hospital and the community. Functions include serving as liaison between administrations, departments, community organizations and/or other city agencies to help ensure access to appropriate services and resources available to patients throughout the hospital and within the community. CLW' s may be assigned to any inpatient, outpatient, or Emergency Service area based on hospital needs.

Tasks may include:

1. Work directly with the social work and casework staff as an extender, to work with patients/families in negotiating systems within the hospital and. in the community.

2. Providing information about community and governmental

resources.

3. Assist with the implementation of discharge plans.

4. Escorting patients into the community and making home visits as needed.

5. Working closely with related social and community agencies.

6. Other tasks that may be assigned.

Minimum Qualifications

1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,

2. Education and/or experience equivalent to (1 above). Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above). However, all persons must have at least one year of full-time experience as described in (1 above).
