Arranged transportation, as required, for residents requiring social services and/or medical care. Provided housing and supportive services, supervision and training of staff, personalized assistance, and/or health-related services to residents with chronic health conditions, illnesses, or disabilities. Served as the initial and primary point of contact for health care and health- related activities. Accompanied residents to dialysis treatments, as or when necessary. Maintained files, medical reports, and records of all residents and reports to the nurse any significant changes in the condition or behavior of residents. Attended to the behavioral healthcare needs of residents experiencing depression, dementia, and other conditions. Administered insulin treatments, blood pressure measurements and blood sugar, as directed by residents’ physicians. Interacts and communicates with visiting nurses, as or when necessary. Reorded medications from the pharmacy for all residents, when medications were almost depleted. Assisted residents with personal care and daily activities of daily living, as necessary.
