Patient’s diagnosis: Left CVA with right side hemiparesis, aphasia, dysphagia, diminished cognitive level and memory loss. Assisted with patients ADLs and IADLs, including urinary incontinence, bathing, dressing, grooming, feeding while grading activities to help patient be independent as possible. Implemented use of adaptive equipment, addressed safety issues, practiced energy conservation while progressing with activity tolerance and strength. Used OTA knowledge and experience to guide patient’s rehabilitation.

Responsible for writing all patients daily notes, choosing activities and exercises and administering tests for children and adults to access patient levels. Case load consisted of 6-8 patients a day with time frames from 45-90 minutes for each patient. Responsible for writing all patients daily and weekly progress notes. Incorporated creative therapy ideas for patients with various diagnoses, such as: CVA, Orthopedic (Back, Neck, Hip, and Knee), Amputations, Cardio Infarction, and debilitating diseases while adhering to standard, droplet, and airborne precautions. Administering tests; vital signs, manual muscle testing, dynamometer, cognitive, and safety. Handled family consultations, patient education, and home exercise programs.
