Burn Unit alone without a Social Worker. I was on my own after starting
position there after 3 days. I was able to adapt quickly to the new hospital

census of 45-50 patients a day. Made referrals through Navi Health to next
level of care. Set up home care and ordered equipment patients needed at

While employed at Mercy I assisted in the development of the Infusion/PICC Team. I also, developed the Home Care Liaison Position and recruited by the Case Management Team. I am proud of my accomplishments and with the development of me professionally. During my twenty-nine plus years of employment at Mercy I maintained a thorough accurate and confidential documentation in the electronic medical records. I advocated for patients by supporting and respecting basic rights, values, and beliefs. Oversaw patient admission and coordinated the discharge planning. I worked with Ensocare as the referral source to other facilities and Cerner for charting. I also worked with Milliman Care Guidelines for determining a patient status.
I trained other Case Managers and worked on the Medical Floor as well as the Critical Care Unit. During Covid I was the only Case Manager in the Critical Care unit. Determining placement and assisting in the transfer of critical patients for ECMO and other life sustaining procedures. Determined next level of care for patients such as to Long Term Acute Care Hospitals, Acute Rehab Hospitals and setting up transfers of patients to other facilities.
