OCR Case Manager - QMHP (Mar 2021 – Present)

Provide competency restoration services, psychosocial rehabilitation services, and service coordination to individuals who have been found incompetent to stand trial. To also provide transitional services to individuals who have been found competent through both the state hospital and Jail Based Competency Restoration and who’s case have been disposed of. Essential Functions reviews all assigned cases, review all new referrals, i.e., the arrest record, and other criminal justice information related to the case upon receipt, check the Avatar for data concerning the client. Contact prior psychiatric service providers, family members significant others, and or other agencies to obtain information on client's psychiatric history, social functioning, and treatment needs, obtain consents for treatment and for release of information from each client referred or document the client's refusal to sign them. Provide feedback to the psychiatrist, supervisor(s), jail staff, and other MHMR staff and or medical staff as needed. Attends all mandatory meetings, including weekly team meeting with supervisors. Provide competency restoration services, which include mental health and substance abuse treatment services as well as legal education for individuals found Incompetent to Stand Trial. Performance Requirement(s). Provide legal education to the client following NTBHA approved curricula based on the individual needs of the client. Provide supported housing for clients served in the program who lack adequate housing. Provide face to face services a minimum of twice to three times per week for clients served in the program to ensure participation, promote adherence to treatment, and ensure the client's basic needs are being met. Provide substance abuse referrals to clients with substance abuse issues within 21 days of being admitted to the OCR Program. Follow up within 24 hours with a client who misses a scheduled appointment with any MHMR staff person. Provide services in compliance with the Resiliency and Disease Management Service Package requirements, including a plan to maintain housing and utilities for three months, ensure the client is connected with an appropriate provider for medication management, ensure the client is connected with a medical provider, and completion of all appropriate benefits applications. Provide continuity of care to clients completing the program. Provide continuity of care to clients who do not complete the program or who are determined to be incapable of restoration to competency. Ensure that a service encounter happens with a client of the program on the day the client will be released from jail. Check medication logs, pills, etc. a minimum of once per week with each client to ensure medication compliance. Provide transitional services to Individuals, previously in a competency restoration program and who’s case have been disposed of. Provide transportation from jail to housing location or bus station upon release from jail. Complete an RFC med request prior to Individual’s release. Connect individual with an MHMR outpatient clinic as needed. Complete ROIs for client’s family and collaterals. Assist in connecting clients with outside resources as needed.
