Personnel Coordinator
Responsible for knowledge and establishment Policies and Procedures willingly and knowing to carry out the published polices of the agency and of all Programs and Home Emergency, Back-Up Plans Evacuation, Supervised Independent Living Programs. Participate in programs of all pre services and in service trainings. Properly maintain all hired individual applications and employees referrals upon interview and hiring forward all to the Director of Personnel. Attend all scheduled staff meetings with Supervisors inform them of staff changes, placements and implement major operational changes for replaced staff. Carefully hire individuals to work with clients with development challenges and work closely with facilitator for each given case load. Work as needed with clients with DSP if not available to work scheduled shift. And perform other health and safety related duties as requested by their facilitator. Contribute to development a positive relationship with all staff placed and with each client and make sure the guardian and client satisfied with ongoing scheduled services.
Program Coordinator/ Facilitator
Responsible for knowledge and establishment of all Policies and Procedures willingly and knowing to carry out the published policies of the agency and maintain the policies and procedures of the Home Emergency, Evacuation and Health and Safety for all Programs. Required attend all meeting for program and services. Address all service plans work closely with case management, clients, and guardians. Attend all mandatory quarterly and annual CPOC for client on assigned case load. Participate in programs of pre –service and in service training, proper maintain service documentation as specified in the standards of Agency. All records and forms will be completed and submitted to case management, Make sure all clients as ongoing services and as assigned qualified Staff that trained and certified to work with clients, family to continue on going implemented service plans that required, Supervisor, intake, and training all assigned DSPs. Practice preventive safely and health care techniques and obtain immediate treatment in case of illness or accident. Make sure all Service plan, Back Up, Evacuation Plans, and all Client Data is current. Mandate hours of training home visits, and all trained type(s) for clients receives per the Plan of Care. Keep a positive relationship with DSPs, Staff, clients, family, and case management.
DSP
Responsible for overall direct support for the person assigned. Attend all needs of the consumer such as personal hygiene, household chores, community outings, Redirect or intervene during behavioral episodes as needed to ensure the safety, health, welfare, and environmental well-being for clients. Mandate and follow all strategies per Service Plan, Care Plan, and Behavior Plan for the particular client. As the DSP must alert to respond to clients’ needs practice fire drills, supervised and monitor client even slipping. Responsible for acting as a skilled trainer based on given clients’ needs, help with cooking, shopping, house maintained and administer medications and or prompt medical devices. Must be also conscious of safety to prevent all incidents or potential hazards. If incident occurs carefully complete CIR and forward to proper authority. Contribute to the development and keep a positive relationship with client, supervisor, families, community, and case management. Report and attend ongoing training and certifications.
