etc.
Serve as the single point of access for patients needing public or private assistance. This provides clients with medical and mental health treatment coverage, food stamps, monetary allowances to cover housing and utility bills, drug and alcohol counseling, clothing, and prescription coverage.
Advocate for clients, assist in applying for social security benefits, welfare assistance, find housing options for clients' and assist in application process, while keeping up to date with deadlines, to prevent delay in the client receiving entitled benefits.
Contact new referrals quickly and efficiently in order to assist clients with proper care and treatment, which is paramount when dealing with clients who are nearly homelessness, or are home, and are dealing with mental health conditions.
Completing progress notes on each client on a weekly basis in order to maintain company standards, and allow for other agency staff to enter into the data system to become up to date with the clients current progress and long term goals.
Complete referrals in a timely and thorough manner to residential facilities within our agency, and to outside mental health agencies, thus ensuring the clients receive the quickest housing placement, and proper level of care for their mental health condition.
Maintain confidentiality regulations in accordance with HIPPA privacy act.
Work in conjunction with other PATH and Administrative Case Managers to assistance other clients with receiving necessary benefits, and care, so that our
clients are able to prosper in the community, and live healthier, more independent lives. Our team of case managers meets weekly to go over our individual clients,
which allows for open communication between the case managers in order to develop better strategies, and plans of action to better serve and care for our
clients.
