• Assist the facility M&E focal person or supporting M&E staff in keeping track of summary data and source documents ( patient/client form, registers and summary forms and assist in updating facility registers and preparation of summary reports during the reporting period and when necessary, that is weekly, and monthly reports
• Ensure that clients’ folders are up to date, that is Smart-Care forms are in each client folder and is appropriately completed
• Update all the facility dashboards and ensure facility performance is reviewed using the data displayed
• Ensure appointment scheduling is done for all clients due for drug pick-up
• Maintain confidentiality of information on source documents or in database
• Ensure that that Viral Load results for clients are accurately updated in Smart-Care
• Work with facility staff and or the M &E person ( whether facility based or support staff) to resolve questions, inconsistencies or missing data and verifies accuracy of data before entry into electronic systems
• Assist in establishing and maintaining an effective and efficient records management system with a good audit trail at facility level
