Job description:

Charge nurse: Supports the Divisional Nurse Manager in leading, precepting and mentoring nursing staffs as the clinical expert in the unit. Key member of the interprofessional team and ensures that the standards for nursing care are adhered at unit level, while working within an assigned shift. He/she is accountable for the shift coordination, guest flow, guest experience and resource allocation and works in cooperation with medical providers to ensure delivery of quality guest care. Delivers clinical care for the majority of their time and supports the Divisional Nurse Manager to ensure successful commissioning. Ensures nursing is practiced in accordance with professional values and moral principles and maintains confidentiality as appropriate.

Care management nurse job: is to assess patient health and needs, follow patient care plan, educate patient about their health and gaps that patient have and evaluate patient to maintain in healthy life style, we worked on 3 programs: ischemic heart disease ( assess and follow up patient disease progress, treatment, medication, complications, provider follow up and appointment ), diabetes program (assess and follow up patient disease progress,treatment, complications,medication,self-monitoring, provider follow up and appointment ), and maternity program(we follow up with women from day of pregnancy till 6 month after birth as postpartum follow up, educate woman about pregnancy care, provider follow up, her lab test and medication, risk for complication,educate about types of delivery, new born care), we gave each patient health education according his program and follow up with patient progress and maintenance, we had internal advance training for each program to deliver high quality of health care to patient.

Registered nurse: provides health and medical care for dialysis patients, the nurse provides expert care to the patients while working with the family to address their concern, fears, problems and options, the work apart of team of physician, and other health care professionals to provide care, monitor health condition, plan long term care needs, administer medicine, use medical equipment’s, and advice patient and their families in their illness. given Health education regarding disease process.
