Intake Specialist-Pool

●Screen patients over the phone as well as possible regarding mental health and/or addiction/withdrawal to help physicians determine best level of care for patient. Patient has determination in process to request IP or an Outpatient program but there are criteria to meet for each specific program. Complete detailed case notes with questionnaire and determine insurance eligibility. If out of network inform patient of non Rogers in network MH sites available or explain out of pocket costs for non Medicaid patients.

●During screening process, use expediting tool if patient meets criteria as SI w/plan or thoughts or as a danger to self or others. These patients will be admitted ASAP to IP program to first available placement made available at anyone of our three locations usually within hours and transportation will be made available when necessary. This may also be a transfer from a hospital facility in which case we arrange ambulance transfer upon physician to physician medical clearance.

●Follow EMTALA guidelines

●Review and Transfer cases. Review all cases with doctors/NP’s and complete hospital transfers from calling hospitals. Obtain recommendations from physician and pass on to beds role for availability check and ultimate scheduling. Complete case notes for every patient/transfer encounter in every role. Ask physician if health and wellness check is necessary before acceptance during review. Physician will determine if patient needs inpatient or outpatient level of care based upon review which is based upon initial screening information. For hospital transfers perform screening as aforementioned.

●Registration Role. Complete registration for scheduled patients and f:f screenings for walk-ins. For registration role admit patient to inpatient unit and review all admissions paperwork with patient and/or patient guardian and have patients sign electronically as proceeding through the process. Inform unit upon patient arrival and patient admittance completion. Track times as well as add case notes and info to admissions tracking log and inform beds role that patient is admitted. Send report via Teams to unit upon patient arrival with patient specifications of MH and any medical conditions.

●Work with team at anytime necessary and be willing to switch roles as necessary.

●As pool be willing to work all shifts and weekends/holidays and fill in for regular staff when possible.
