Presenting: QUALITY OF PEDIATRIC CARE OF MY PEDIATRIC PATIENTS.

I present 3 cases, children I saw for the first time I always do a complete examination even if they arrive for ear pain or sore throat, etc.

CASE No 1 Female child 9 years old, since born she was seen in the Pediatric clinic, arrive with sore throat with history of Asthma.

At Exam – purulent secretion on tonsils, lungs few sibilants, Heart has a SYSTOLIC MURMUR MITRAL AREA, Abdomen normal.

I ask the mother if she had a Heart Murmur when she born? Mother says NO

Explained to the mother my finding- The tonsillitis will be treated with antibiotics and the Heart Murmur need to have a 2 D Echocardiogram that is the “Gold Standard” for Diagnostic between Physiologic and Pathologic Heart murmur.

The mother does not believe me, she mention I brought my child since born to this Pediatric Clinic and never was mention about her Heart Murmur.

I ask the Mother if she wish to be referring her daughter to the Cardiologist for 2 D echocardiogram.

Mother answer Yes, but I do not believe she has any heart condition.

Refer to Cardiologist for 2 D echocardiogram, because she was asymptomatic, no cardiac sign the Cardiologist do not want to do the 2 D echocardiogram.

Mother insist and the Cardiologist did a 2 D Echocardiogram

The assistant after the 2 D Echocardiogram arrive fast and inform to the Mother, the Cardiologist want to speak to you.

Cardiologist:” I want to inform you that the 2 D echocardiogram is ABNORMAL:

Right Atrium moderated dilated.

Left Atrium is Normal

There is an ATRIAL SEPTAL DEFECT WITH LEFT TO RIGHT SHUNT -The defect measure 10.1 mm. The tricuspid valve with mild INSUFFICIENCY (Peak gradient of 19.0 mmHg), Mitral Valve Normal, and Right Ventricle is MODERATE ENLARGE

The Cardiologist inform the Mother that her daughter needs to have an open heart Surgery to close the hole in the Heart-

The Cardiovascular Surgeon inform the mother, if nobody found the heart Murmur at age 15 the child will die of irreversible Pulmonary Hypertension and the “Asthma “was related to the blood moving from the left side of the heart to the right side of the heart increasing the volume and pressure in the Pulmonary vessels and leak of fluid on alveolus, bronchospasm and producing a PSEUDO ASTHMA.

Child was schedule for Open Heart surgery and a patch on the hole of the heart was done child doing well no more asthma no more medication for asthma, follow by cardiologist.

Mother and child arrive to the Pediatric Clinic to thank me because I found the heart murmur and save her life.

CASE No 2 .- Child 9 years old, new patient arrive to the Pediatric clinic with the History of Chest pain at 2:30 p.m. after running in the School and need to stop .

Mother informs that she mention many times to the doctors, about the chest pain after exercise in School, but nothing was done.

Examination. - Child has Pectus excavates, a SYSTOLIC MURMUR MITRAL AREA AND A BRUIT ON THE SUPRACLAVICULAR AREA BASE OF BOTH CAROTID ARTERIES AND ARCH OF AORTA.

I refer to Emergency room Children Hospital requesting 2 D echocardiogram for possible Arterio/venous fistula (bruit) - in ER nothing was done; only they prescribe Tylenol.

Mother arrive next day to the Pediatric Clinic and I refer to Cardiologist for 2 D echocardiogram, the Cardiologist does not want to do 2D echocardiogram, because is expensive for Medical, the mother get mad and insist and 2 D echocardiogram was done

REPORT: FISTULA BETWEEN PULMONARY ARTERY AND CORONARY ARTERY.

The chest pain was produced because the Pulmonary artery “steal” blood from the coronary artery and the minimal flow of blood in the coronary artery was not able to give OXIGEN to the heart muscle, producing ISCHEMIA of the Myocardium and producing chest pain when running. - Schedule for future CARDIO VASCULAR SURGERY.

CASES No 3. - 2 brothers 9 years old and 12 years old arrive for the first time to my Pediatric clinic in Texas.

a.- 9 years old boy refer sore throat during the exam I found pus on tonsils and a Systolic murmur Mitral area receive treatment with antibiotics and refer for 2 D echocardiogram – has a large opening in the Atria Septum – the cardiologist close the hole in the heart with an “umbrella” doing well and follow by Cardiologist.

b. - 12 years old boy. - Also arrive with sore throat, found pus on tonsils and treated

found A SYSTOLIC MURMUR AORTIC AREA. Had history of back pain and seen by orthopedic. Refer to Cardiologist for 2 D echocardiogram – report DILATED AORTA ANEURYSM, the genetic test report - Atypical MARFAN SYNDOME receive treatment with antihypertensive medication, and follow by cardiologist and future AORTIC SURGERY.

The back pain was related to the dilatation of AORTIC ARTERY.

For your information my PhD Thesis is about “Heart murmur in children in Fabens Texas”, found 592 children with heart murmur all were referred to the Cardiologist for 2 D echo cardiogram - The report of the 2 D echocardiogram :

a. - Pathologic Heart murmur 300 =50.68 %

b. - Physiologic Heart Murmur 292 = 49.32 %

I made an statistic analysis and develop 2 protocols.

120 children was placed in SBE precaution Protocol other children had Cardiovascular Surgery.
