Gerontology Medicine consult service Michigan Medicine-Ann Arbor, MI (2nd Rotation)
125 hours of clinical experience in gerontology medicine consult services; performed primarily neurological, psychological, and medical assessment and interventions. The request of this consult service was available to the entire inpatient Michigan Medicine Health System. Typical patients seen by this service had the following diagnoses and conditions: Cancer, CAD, Failure to Thrive, LVAD, MVC, AKI, Fall, Trauma, Pneumothorax, CVA, Post Orthopedic Surgery, Post Extubation, HF, Sepsis, Abdominal Aortic Aneurysm. The service can be requested or triggered by protocol (age and situation).
A typical day for an NP on the service includes:
oExamine and assess acutely confused elderly (>65yo) patients with and without history of dementia.
oAssist and guide medical decisions regarding medication and dosing adjustment (polypharmacy) for the geriatric populations (follows guidelines and recommendation such as Beers geriatric criteria)
oAssist in safe discharge planning and follow up for certain vulnerable population
oNavigate and collaborated with hospital legal experts regarding guardianship issues and evaluating patient capacity for medical decision making
oDaily follow up with patient and family
oAssist family with end-of-life decisions, collaborate with palliative services
oPrimary Diagnoses: Delirium, Fall, MVC, Syncope, Sepsis, HF, LVAD, Polypharmacy, Hip Fracture, Elderly neglect, head injury

250 hours of clinical experience. This clinical experience provided me with the opportunity to encounter a variety of common suburban emergency complaints such as chest pain, DIB, stroke, PE, DVT, pneumonia, back pain, abdominal pain, male/female GU/GYN complaints, UTI, URI and common injuries.

JOB DESCRIPTION
Nurse practitioner seeking a position in an inpatient/outpatient setting, that requires higher education and advanced practice training to collaborate with other disciplines, utilizing excellent communication, organization skills, and strong work ethic. In my Nurse practitioner career, I see myself working with a hospitalist team, specialty team, or procedural team. I believe there is joy and fulfillment in all these settings and career choices. Most importantly, I would like to be able to collaborate and independently practice when appropriate. My training and nursing experience were focused on acutely ill patients, I would like to remain in this environment and setting.
In my career I would like to be able to perform the following:
Complete a thorough H&P, differentiate illnesses, establish diagnoses and plans, treat accordingly
Order appropriate laboratories, radiological studies and other routine diagnostic procedures to identify pathophysiologic processes
Perform specialty routine procedures, depending on specialty service joined
Educate patients regarding their disease process, treatment plans, potential adverse effects, and management of these side effects
Maintain primary management of patients on inpatient unit
