My responsibilities were mostly adult patients from age eighteen to the geriatric population who are acutely ill with a wide variety of medical issues providing bedside care for a wide variety of medical patients, including pre- and post-surgery patients.

Duties of a Charge Nurse and a preceptor for newly employed Nurses

Administering medications with appropriate doses, prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications, Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications, notifying doctors of any discrepancies noted.

Coordinating care as appropriate with pre and post-surgical care, educates patients on surgical procedures, monitoring for adverse reactions after surgery, apply dressing changes as appropriate and specified by the surgical team.

Monitoring and care of all drains, monitoring and documentation as appropriate

Managing intravenous lines with central line sterile dressing changes, Foley catheter changes, insertion and removal with necessary documentation.

Blood administration, observing and monitoring patients' conditions, records patients’ medical information and vital signs maintaining records and communicating with doctors

Prepare significant change forms in the event of a change in condition, notify MD and family. Utilizes knowledge of current health/illness management and research findings in developing and providing education to clients and family members/significant others.

Prepare Occurrence report, and fall investigation sheet for falls; prepare skin sheets if appropriate

Check lab results, and notify MD for abnormal results, fill out lab log upon receiving lab results. Note changes of Medication or Treatment on MAR or TAR as appropriate

Document specifically on patients especially on Medicare and Managed Care Residents. E.g. when documenting on why prn antipsychotic was administered, documentation includes specific behavior like biting, kicking, spitting, etc. not just writing agitation

Initiate skin sheets for skin tears, rashes, pressure ulcers, and bruises. Skin sheets are updated weekly

Monitor rotation of nurse’s carts weekly

Supervise meal times, and assist with feeding as necessary making sure that all precautions especially aspiration precautions are in place

Ensure nail care, mouth care, showers, toileting and repositioning as appropriate

Ensure control forms for narcotics are signed by MD’s in a timely manner

Respond swiftly to complaints of pain by assessing for swelling, bruising, discoloration, Range of Motion, and follow up with appropriate treatment and documentation.

Facilitate completion of weekly weights weekly and monthly weights. Notify MD and dietician of any significant weight changes.
